Telemetry of programmed and measured data is an important feature of many pacemakers currently used in clinical practice. The ability to receive non‐invasive data from the implanted device constitutes a major advantage for the long‐term follow‐up of the patients and of device performance. There are numerous types of data retrievable via telemetry: parameters of device characteristics (output, battery longevity, impedance, etc), event recorders or counters, event markers, and endocardial electrograms. 摘要:
An automatic computer generated Pacemaker Diagnostic Diagram described previously (PACE 8:691) was plotted below the ECG to aid the diagnosis of complex pacemaker‐heart interaction. During routine followup visits, 14 patients with Symbios 7005 or 7006 DDD pacemakers were studied during various temporary programming protocols to demonstrate common and unusual pacemaker problems. The examples discussed in this paper explain how the side‐by‐side ECG and diagram synergistically aid human diagnosis of undersensing, oversensing, loss of capture in either or both chambers, upper rate limiting, safety pacing and atrial arrhythmias. 摘要:
The facility of direct real‐time endocardial electrogram recording offered by newer pacemaker models can be helpful in the assessment of normal pacemaker function. Confirmation of the main hallmarks of sensing (amplitude, slew rate, and timing of the electrogram) can be achieved. Assessment of pacing capture also can be made; techniques for further analysis using external signal averaging can enhance this. The measurement of atrial and ventricular electrograms by telemetry has led to alterations in the protocol used at implantation to allow for the input impedance of the sensing circuit of the pacemaker. Accurate measurements of retrograde VA conduction times and confirmation of normal upper rate limit behavior of ODD pacemakers can be achieved simply and reliably using telemetered electrograms from the permanent pacemaker syste

Some pacemakers are capable of storing in memory and then, upon command, reporting the frequency of pacing and sensing in either or both cardiac chambers as well as other features of pacetnaker function. Rate and possibly sensor histograms can be generated automatically by the system to facilitate an assessment of its behavior over a period of time. Although available in some single‐ chamber Vitatron pacemakers and the Siemens‐Pacesetter Sensolog, this paper reviews the utility of this technology in both single‐ and dual‐chamber Intermedics units in the assessment of programmed rate and AV interval parameters and overall pacing system function. Increasing numbers of pulse generators have one or more features that assist in the diagnostic evaluation of the pacing system. Interrogation of programmed parameters, measured lead and battery data, telemetry of event markers, endocardial electrograms, diagnostic data concerning the frequency of pacing and the presence or absence of spontaneous arrhythmias all facilitate the ease with which the pacing system is evaluated and the patient managed. The utility of these features is illustrated from five separate cases demonstrating the value of the individual features and how they may complement one another in extremely complicated pacing system problems.

In order to demonstrate the feasibility of dysrhythmia analysis by means of noninvasive intracardiac telemetry, a custom‐modified AFP DDD pulse generator was attached to temporary pacing electrodes in 15 patients undergoing electrophysiology studies for a variety oftacliy‐ or bradyarrhythmias. Intracardiac atrial and ventricular electrograms were obtained during induction, termination, and drug intervention in patients with these arrhythmias. The telemetered computer‐generated hardcopy tracings compared favorably to those obtained through a standard multichannel physiological recorder, and thus confirmed the accuracy of the technique. A growing number of patients require invasive electrophysiological testing initially to evaluate a predisposition of symptomatic tachyarrhythmias. Once confirmed, periodic repeat testing is required to assess the status of the arrhythmia and the effectiveness of any therapeutic intervention. Many of these patients also have a single‐ or dual‐chamber pacemaker for bradycardia support.
