AbstractThe acceptance of fine‐needle aspiration biopsy in the diagnostic work‐up of pulmonary masses has resulted in an increased number of neuroendocrine tumors of the lung first encountered as aspiration cytology specimens. The accurate cytologic identification of these neuroendocrine neoplasms is important in that they have relatively specific clinical and prognostic features in contrast to nonneuroendocrine neoplasms. We report on the cytologic features of 46 primary pulmonary neuroendocrine neoplasms initially encountered on fine‐needle aspiration biopsies. The neoplasms are separated into three distinct cytologic groups, including the typical carcinoid (13 cases), the atypical carcinoid (3 cases), and small‐cell carcinomas (30 cases). The clinical features of all cases—and histologic findings when they were available—are also considered. Finally, a detailed cytologic description of the three groups of neuroendocrine neoplasms is presented with emphasis on differential diagnosis including nonneuroendocrine pulmonar

AbstractThe cellular changes produced by radiation therapy were studied in cytologic specimens from seven patients with bronchogenic carcinoma: squamous‐cell carcinoma (four patients), adenocarcinoma (two patients), and small‐cell undifferentiated carcinoma (one patient). Cytologic samples were obtained from sputum and bronchoscopic examination in patients before and after they received a course of radiation. The cells were studied by light microscopy. An increase in the percentage of columnar cells was detected in five of seven patients independent of the latency of cytologic sampling posttherapy. The percentage of macrophages was unchanged in patients sampled at 15, 18, and 24 mo after the last radiation dose; leukocytes were decreased in relative frequency in the majority of patients studied. Following irradiation, moderately atypical and severely atypical metaplastic cells were increased in relative frequency (48% versus 15% and 19% versus 3%, respectively) and malignant cells were decreased (40% versus 22%) or absent in all patients, irrespective of the type of carcinoma. Nuclear vacuolation, nuclear enlargement, loss of chromatin texture, and rupture of the chromatinic rim were seen in atypical and cancer cells. Infiltration of cancer cells by leukocytes was observed in two of four patients with squamous‐cell carcinoma. AbstractAdequate sampling is important to minimize the risk of false‐negative fine‐needle biopsy (FNB) diagnoses. This article reports a prospective study of the following criteria for adequacy of sampling for FNB benign diagnoses: at least six aspirates, at least two of which each contain at least six clusters of benign cells. Of 888 patients studied with FNB, 60 had FNB diagnoses of malignancy; 58 were surgically confirmed, and there was one false positive. Seven of eight nodules with diagnoses of suspected malignancy were excised; three were malignant. Fifty‐seven nodules had diagnoses of cellular or Hurthle‐cell adenomas; 35 were excised, and four were malignant. All 33 nodules excised after benign diagnoses (including Hashimoto's thyroiditis) were benign. AbstractAChlamydiadiagnostic service has been established as part of a cytology laboratory, combining prediction of possible infection by routine cytology with definitive testing by immunofluorescence. The first 10 mo of operation have had good clinician acceptance with 527 specimens submitted and 128 reported as positive (24.3%). Routine cytology was more accurate in predicting chlamydial infection than was clinical evaluation (32.8% of cytologically suspected cases were positive versus 20.2% of clinically suspected cases); however, both failed to identify many infected women. The patient selection for testing must be improved since only about 22% of infected women in our population were tested. Nevertheless, the service identified a large number of patients infected with an important sexually transmitted pathogen. AbstractThis pediatric cytological and clinical study aimed at assessing the value of nasal eosinophilia during nasal provocation tests for identifying an offending allergen. The population studied comprised 50 children aged from 4 to 18 yr; 39 of these had well‐characterized allergic rhinitis, which in 21 cases was combined with asthma, and the remaining 11 had nonatopic chronic rhinitis. Nasal secretions, collected by nose blowing, were stained with May‐Grünwald‐Giemsa or Wright stain. The percentage of nasal eosinophils was obtained by examining the cells in the whole slides. Counts were carried out on secretions collected before challenge, after insufflation of saline solution (for verification), and 40 min after insufflation into each nostril of an allergen (housedust mite extract). The nasal provocation test was considered positive when insufflation of the allergen increased nasal eosinophilia by more than 10%, provided that the prechallenge proportion of eosinophils was less than 50%. AbstractBronchial brushings to evaluate cytopathologic changes were collected from 28 patients exposed to a toxic atmospheric level (66 parts/million) of chlorine gas. Postexposure smears collected on day 5 showed basal‐cell and goblet‐cell hyperplasia, acute inflammation, and chromatolysis of columnar epithelial cells. Columnar epithelial cell syncytia were observed in 15 (53.57%) smears. Nine (32.14%) smears showed abundant nonpigmented alveolar macrophages. Seven (25%) smears from mucosal erosions showed proliferating fibroblasts and capillary fragments; on day 15 and day 25 repeat smears from these seven cases showed evidence of epithelial regeneration and repair byfibrosis. AbstractEleven cases of rare papillary carcinoma of the breast diagnosed by fine‐needle aspiration cytology (FNAC) are reported. Five of these were pure papillary carcinomas and six were mixed papillary and ductal, lobular, or mucinous carcinomas. AbstractThe efficacy of utilizing immunocytochemical staining of prostatic basal cells in separating benign from malignant prostatic epithelium was tested by staining fine‐needle aspiration smears of prostatic lesions with the monoclonal antibody EAB‐903. This antibody has been shown to stain keratin subtypes present in the prostate only in basal cells. The study utilized 12 benign, nine malignant, and four suspicious‐for‐carcinoma cases. AbstractSteroid hormone receptors were studied in 45 patients with primary, recurrent, or metastatic ovarian cancer in cryostat‐frozen sections and imprint preparations. The ligands, 17 B‐estradiol‐6‐carboxymethyloxine‐bovine serum albumin fluorescein isothiocyanate (FITC‐BSA estradiol) and hydroxypro‐gesteronehemisuccinate bovine serum albumin tetramethylrho‐damine isothiocyanate (TMRITC‐BSA progesterone) were used in the fluorescent cytochemical method. Results were compared with standard dextran‐coated charcoal (DCC) biochemical assay. An overall significant correlation between biochemical values and cytochemical results was found. AbstractCancer of the uterine cervix is the most common malignancy among Indian women. Evidence has shown that cervical cancer is preceded by dysplastic, or precancerous, lesions. HSV‐II has been associated with this process of cervical carcinogenesis.
