&NA;One hundred muscle flap transfers performed at the Mayo Clinic from 1975 to 1978 are reviewed and assessed as to the cause and location of the defects, muscles used, complications, and results. We found the muscle flaps very versatile for covering for a wide variety of difficult soft tissue and bony defects. The complication rate was very low, considering the severity and chronic nature of the problem. Ninety‐two percent of patients showed healing after surgery, and 82% remained healed at the time of this follow‐up survey.

&NA;The authors describe a simple method for surgical correction of senile entropion that is a modification of the jones, Reeh, and Tsujimura technique. The modification consists in the use of the classic blepharoplasty incision, dissection of a skin flap, and resection of a strip from the orbicularis oculi. After the resection of excess skin, suturing begins— passing through skin, septum, and skin, correcting the deformity on the lash border.The anatomy of the region is discussed, as well as the etiology of the condition and preoperative management. The result of this technique in 26 patients with senile entropion is described. There were no complications after twelve‐ to thirty‐month follow‐up, save in 1 case in which a hematoma occurred. This was subsequently drained.

&NA;Two new types of silicone mammary‐type implants designed to reduce silicone leakage by a gel barrier or to insulate tissue from silica in standard implants (by silicone coating) were placed in rabbits. Standard‐type silicone implants were placed as controls. Capsules histologically similar to those that occur in humans occurred around many implants, although no grossly visible capsular contraction occurred. Inflammation was minimal around all types of implants, and the capsules appeared similar. Myofibroblasts were seen around implants within the first three weeks only.Scanning electron microscopy with energy‐dispersive x‐ray analysis was used to positively identify silicon in tissues around the implants. Silicon was found in tissues from 55% of the standard and 60% of the silicone‐coated implants, while it was found in only 10% of capsules around implants containing a gel barrier layer.We conclude that both kinds of new implants incite minimal reaction in rabbits, and that a gel barrier can reduce silicone leakage from silicone implants.

&NA;A simple, practical alternative to the traditional method of excising small cutaneous lesions is described. It has the advantages of decreased length of final closure and flexibility in determining the orientation of final incisions in areas where it may be difficult to accurately determine skin tension lines.

&NA;Historically, treatment of the burned female breast has been unsatisfactory. By using the latissimus dorsi myocutaneous flap, many patients can be reconstructed in a single operation. A more aggressive approach in adolescent patients whose breasts are developing may result in more normal breasts. In such patients, nipple reconstruction is delayed until a later time.

&NA;We have found the tensor fascia lata myocutaneous flap to be exceedingly reliable with a consistent vascular pedicle; it can be elevated easily and rapidly in its subfascial plane. It is a useful flap in the paraplegic patient for closure of ischial, trochanteric, and sacral pressure sores, and may also be used as a free flap with little donor morbidity. At present, we recommend this as the flap of choice for closure of trochanteric pressure sores. Additionally, it appears to have further applications in defects of the groin, perineum, abdominal wall, and lower chest wall.Eleven tensor fascia lata musculocutaneous flaps, utilized for soft tissue coverage for varying defects in 9 patients, are discussed.

&NA;On the basis of 35 maxillary osteotomies of the Le Fort I and III types, the authors discuss the main problems connected with treatment of maxillary hypoplasia and midfacial retrusion. Special attention is paid to cases that demand coordinated treatment including skeletal as well as soft tissue repair and reconstruction. Thanks to combined treatment in all cases, including 7 patients with partial backward displacement of the osteotomized fragments whose occlusion could not be entirely corrected, striking or highly satisfactory improvement of appearance was achieved.

&NA;The amputated lower right extremity of an 11‐year‐old girl, severed 10 cm above the knee, was reimplanted on New Year's Eve, 1978. Three skin grafting procedures were required.Seven months later, all wounds healed. There was callus formation at the fracture site and soft tissue contracture at the amputation level. The patient is up and about on a partial weight‐bearing brace with a molded foam shoe to protect her foot. There is excellent circulation in the entire lower extremity.At 6 months postinjury, cable sural nerve grafts for sciatic nerve repair were done elsewhere. This will be the subject of a subsequent report.
