Successful treatment of massive degloving lower extremity injuries requires a flexible approach with adherence to basic surgical principles. There should be aggressive therapy with extensive debridement guided by direct observation and fluorescein vital dye staining. Wound closure, either primary or delayed, should be carried out using autogenous tissues when local and systemic tissue conditions permit.

A wide variety of deformities are seen by the craniofacial team. To facilitate the coding and retrieval of data collected, a system has been devised in which an attempt is made to transfer different clinical problems into units which can be processed more efficiently and encoded more exactly. The different surgical manipulations used form the basis of this system. Careful forethought and understanding are prerequisite for the utilization of this system by the craniofacial team.

The Ivy loop is a simple, safe, expeditious, multistaged procedure for closing large through-and-through defects of the cheek. It permits three-layered closure with good bulk and minimal deformity at the donor site.A cervical tubed pedicle is migrated into the defect in stages, looping the pedicle around the defect. The loop is then opened eccentrically on its inner rim and closed in layers. The broad-based attachment around the rim of the defect minimizes the danger of tissue loss in irradiated areas. Credit for this procedure must be given to Dr. Robert Ivy.The Ivy loop has its place in selected cases. Two examples are presented.

When the flexor tendon sheath must be opened to release a Dupuytren's contracture of the finger, one is sometimes left with an exposed flexor tendon. If the initial incisions in the finger are made to create a proximally based flap, adequate skin is left to cover the defect in the flexor tendon sheath and a split-thickness skin graft can be placed distally where the sheath has not been violated.

The 60-degree two-flap Z-plasty is extremely useful and versatile. Its success depends upon accurate construction of the flaps. In the past, the intraoperative measurements required have been either quite involved or quite inaccurate. We present a simple but accurate method for planning the 60-degree two-flap Z-plasty.

A large craniofacial defect resulting from a close-range shotgun blast is described. It involves the left hemimandible, the left maxilla, the hard palate with nasal support, the roof of the left orbit, and the left side of the oral lining. The defect was reconstructed using autogenous ribs for the facial skeleton. The remainder of the defect was repaired with a composite free tissue transfer of gastric wall and greater omentum on the right gastroepiploic vessels. The gastric wall was used to reconstruct the oral mucosa, the omentum was packed around the bone grafts for nourishment, and a split-thickness skin graft was placed on the outside surface. The technical aspects of the procedure are described and a two-year follow-up presented.

The design of the tensor fascia lata myocutaneous flap can be modified, allowing its use as a V-Y retroposition flap for treatment of trochanteric pressure ulcers. This technique eliminates the rotation point, puts the thickest portion of the flap over the defect, and obviates the need for skin grafts. It has been successfully employed to treat 11 trochanteric pressure ulcers in 10 patients in the past two years.

We present two new flaps, a pedicled one and a free one, for the treatment of baldness. The aim of these procedures is to have the hair lying forward at the frontal region, thus providing a more natural appearance and concealing the anterior pilous implant line.
