A pennant–shaped “SMAS sling” is developed from the preauricular SMAS, leaving it attached inferiorly between the level of the earlobe and the angle of the mandible. This is rotated posteriorly over the mastoid process, tension is applied, and it is secured with nonabsorbable sutures to the mastoid fascia, thereby providing a backward and upward lift to a very secure area. Minimal problems and complications occurred in 34 patients over the past four years.

Seven measurements and two qualities of the soft nose and nasal bridge were recorded in 156 healthy young adult North American Caucasians. Nostriln–type asymmetries and alar base dislocations (31 and 38 of 156, respectively) were associated with other minor nasal disfigurements more often than symmetrical nares or alar base levels. A true alar base dislocation between alae of the same shape was distinguished from pseudodislocation caused by differing alar configurations (two–thirds of cases) or configuration differences along with dislocation (one–third of cases). Columellar deviations, columellar length asymmetry, and alar base dislocations were more frequently to the left, whereas nasal bridge deviations and septal dislocations were more often to the right. On the left side the extent of columellar deviation (5.9 degrees) and degree of alar base dislocation (1.5 mm) were significantly greater than on the right (4.0 degrees, p < 0.05, and 1.0 mm, p < 0.01, respectively). Quantitative analysis of the relationship of the minor nasal disfigurements offers valuable data for both surgeons and clinical geneticists.

A simple technique for correction of a gummy smile by partially transecting the levator labii superioris, the major lip elevator, and decreasing its cephalic excursion using an implant spacer is presented. Results are given for 21 patients, and 3 representative patients are discussed, in whom a silicone implant with maxillary augmentation with concomitant rhinoplasty; cartilage from the nasal septum with concomitant rhinoplasty; and a silicone implant independent of rhinoplasty without maxillary augmentation were utilized.

Gel–filled miniprostheses were implanted in rats, recovered, and reimplanted in fresh animals. Experimental contracture characterized by severe, visually detectable distortion of hemispherical prosthesis mounds into pointed ovoid structures oriented laterally was observed. This finding confirms the existence in rats of a phenomenon similar in appearance to clinical contracture in humans. The incidence of contracture was higher than in the first implantation of these prostheses, which suggests that some property of the prosthesis is critical in the occurrence of contracture, and that this property can be altered by prolonged implantation in the tissue so that contracture is more likely. Implants that were overfilled by injection of extra fluid into the gel showed no contracture, but capsules around these implants did not differ from contracted capsules in protein, collagen, glycosaminoglycan composition, weight, thickness, histological appearance, or incidence of myofibroblasts.
