&NA;We conducted a critical analysis of the available epidemiologic investigations on the causal relationship between asbestos exposure and laryngeal cancer. A review of nine casecontrol studies indicates that the estimated risk (odds ratio) attributable to asbestos exposure alone is negligible when smoking and ethanol intake are appropriately controlled for. Six of the 12 cohort studies demonstrated no significant increase in the standardized mortality ratio due to asbestos exposure. The remaining six longitudinal studies showed an increased standardized mortality ratio from 1.91 to 5.41 but no adjustment was made for the confounding effects of smoking and ethanol consumption. In conclusion, the available epidemiologic evidence does not support a causal association between asbestos exposure and laryngeal cancer.

&NA;For more than 60 years the US Navy has maintained occupational health programs for its civil service workers in shipyards, arsenals, and aircraft repair facilities. The early history of the organization, people, and professional activities dedicated to the health of this large federal industrial workforce is examined.Early efforts were stimulated by increasingly complex naval technology and worker compensation law. During World War II training, clinical, and preventive programs were pursued vigorously. Navy occupational health paralleled and at times led the development of occupational medicine and industrial hygiene in America.

&NA;A review of the relevant literature provides evidence that cigarette smoking causes microscopic diffuse interstitial pulmonary fibrosis in humans at autopsy. The association shows a dose‐response relationship and the fibrosis is often severe. Experimental animal studies have confirmed these observations when the exposure to inhaled smoke was prolonged or heavy. Six published and one unpublished cross‐sectional chest x‐ray surveys of adults have shown low profusion of small irregular opacities related to smoking with a dose‐response relationship when this has been studied. Although information on radiographic‐pathologic correlation is deficient, these studies support the hypothesis that smoking causes diffuse interstitial pulmonary fibrosis which is radiologically visible with low profusion in low prevalence. The x‐ray manifestations may be confused with early pneumoconiosis such as asbestosis.

&NA;Mammography was recommended to 212 women, according to American Cancer Society guidelines, at the time of a work site‐offered periodic health exam (PHE). Sixteen weeks later telephone follow‐up determined compliance. A total of 56 (26%) had complied. A second phone contact to 82 noncompliers identified 18 additional women who complied after the first phone contact. Overall compliance (36%) was associated with the presence of physical findings upon breast examination (fibrocystic disease). Compliers did not differ from noncompliers with regard to age, time between PHE and follow‐up, or insurance coverage. There was no association between compliance and age greater than or equal to 50 years or previous mammography.

&NA;This paper presents a new statistical approach for actively using information commonly available in medical surveillance systems to monitor the incidence and severity of industrial injuries on particular operations. The procedures require the practitioner to track the exposure time between injuries. An “out of control” situation is detected when the test statistic exceeds an optimal decision limit, which is determined by minimizing the total injury costs per exposure hour on each operation. The use of this test is presented in the context of a case example from an industrial assembly operation.
