摘要:
PURPOSE.To gain insight into the experience of living with and dying from AIDS and the implications regarding quality of life for patients with advanced disease.METHODS.Exploratory, descriptive qualitative study of 17 patients diagnosed with advanced AIDS.FINDINGS.Patients with AIDS experience physical suffering with changes in their fighting spirit, psychosocial suffering with shattered dreams and a move toward self‐acceptance, and spiritual suffering with the opportunity for renewal. They transcend their suffering and protect and promote the quality of their lives by developing a positive philosophy of life, accepting support, and releasing their creative energies.CONCLUSIONS.The perceptions and experiences of participants provide a holistic perspective regarding their complex needs, and reinforce the importance of a comprehensive approach to care to protect and promote quality of life.IMPLICATIONS FOR NURSING PRACTICE.To alleviate the physical, emotional, social, and spiritual suffering of these patients and support successful coping with their illness, a palliative care approach is recommended. Palliative care, offered by an interdisciplinary team of health professionals, may assist patients with AIDS in affirming life, finding meaning in their illness, relieving suffering associated with the illness and its treatments, and providing the emotional and spiritual support to find comfort and strength even when faced with a life‐limiting illness.Search terms:AIDS, quality of life

摘要:
PURPOSE.To explore the roles of fathers and siblings in families with dying children at home.METHODS.A purposeful sample of 4 fathers and 8 siblings caring for children affected by terminal illness. Ethnographic data was obtained from 4 families over a 2‐year period through volunteering in families' homes and through participating in children's hospice team meetings.FINDINGS.Fathers' roles were described as wager earner, technical caregiver, vigilant advocate for the ill child, and sibling caregiver. Siblings' roles were described as sibling caregiver and school achiever. The third sibling theme was related to emotional expressions of anger and resentment.CONCLUSIONS.Fathers and siblings are affected by the child's terminal illness. Their reactions and perceptions influence how the family functions on a daily basis.IMPLICATIONS FOR NURSING PRACTICE.Understanding fathers' and siblings' roles helps healthcare providers assess, assist, and make plans.Search terms:Fathers, hospice, qualitative research, roles, siblings
