The aim of this study was to register and compare clinical oral mucosal lesions and gingival recessions associated with the use of two different smokeless tobacco products, loose snuff and portion‐bag packed snuff. Selected for the study were 252 men (mean age 36.3 yr) of whom 184 (mean age 36.0 yr) used exclusively loose snuff and 68 (mean age 36.9 yr) exclusively portion‐bag snuff. Oral mucosal lesions were registered according to a four‐grade clinical scale. There was a significantly larger proportion of less pronounced lesions, Degrees 1 and 2, among the users of portion‐bag snuff compared with the users of loose snuff. This was also valid when differences in consumption data were considered. Smokeless tobacco‐associated gingival recessions were found in 42 (23.5%) subjects among the users of loose snuff and in 2 (2.9%) subjects among the users of portion‐bag snuff. Several reports have mentioned the possibility of misdiagnosing pseudoepitheliomatous hyperplasia (PEH) of the overlying mucosa of a granular cell tumor as a squamous cell carcinoma (SCC). Because of this, morphometry was applied to five granular cell tumors with PEH and five well‐differentiated SCC of the tongue. In addition, ten normal tongues have been examined. The mean area, the mean perimeter and the mean diameter of the 50 largest squamous epithelial nuclei in 50 fields were found to be significantly larger in squamous cell carcinomas than in granular cell tumors and normal tongues. Epithelial dendritic cells (EDC) were examined in human oral tissues with non‐specific keratosis, lichen planus and squamous cell carcinoma. Acetone‐fixed frozen sections were stained using an indirect immunoperoxidase technique and monoclonal antibodies to the human CD1 thymocyte (OKT6) and HLA‐DR antigens. Significantly more T6+and DR+EDC were present in lichen planus tissues than normal controls, tissues with non–specific keratosis and the epithelia overlying/adjacent to squamous cell carcinomas, the latter tissues having comparable numbers of both T6+and DR+EDC. By contrast, significantly fewer T6+EDC and significantly more DR+cells were present in the invasive epithelium of squamous cell carcinomas than the overlying/adjacent epithelium of carcinomas, the non‐specific keratosis group and the normal tissues. 23–60% of pathological tissues had either focal or general DR+reactivity in keratinocytes, but there was no correlation between the density of T6+or DR+EDC and the keratinocyte DR status of the tissues. Reports from India showing comparatively lower prevalence rates of oral leukoplakia and lower malignant transformation rates than in western countries appear paradoxical because of high incidence rates of oral cancer in India. On a closer examination of available data however, it becomes clear that there is no inconsistency.

Recurrent aphthous stomatitis (RAS) is a common oral disorder, however, despite detailed clinical, immunologic, hematologic and microbiologic investigation, the etiology of RAS remains unknown. At present, topical steroids and antimicrobial mouth rinses are the mainstays of treatment, but there is still no means of preventing recurrence of the oral ulceration.

Oral hairy leukoplakia (HL) has been regarded as an early sign of HIV infection, and its clinical importance related to the poor outcome of the patients has been emphasized. Initially, HL was observed exclusively among male homosexuals, but subsequently demonstrated in all risk groups of HIV infection. The patient described in this article suggests that oral HL is not specific for HIV infection per se, but may be associated with immunosuppression also due to other causes. We describe an HIV‐seronegative, heterosexual man suffering from an acute myeloblastic leukemia, who developed clinically and histologically typical HL while on cytostatics. Biopsy showed areas with characteristic ballooning cells, and hyphae of yeasts were demonstrated with PAS–stain. Oral hairy leukoplakia (HL) has been seen exclusively in those infected with HIV or at risk for AIDS. This case report describes an example of HL seen in a renal transplant recipient who was negative for HIV on serology and culture. The diagnosis of HL was confirmed using in situ hybridization for EBV DNA.

Twelve biopsies of clinically normal oral mucosa taken from HIV seropositive patients have been investigated by means of light‐ and electron microscopy. Vascular abnormalities were found in all biopsies, regardless of the clinical stage of the HIV infection.
