This study was carried out to investigate the retention of several fissure sealant materials in occlusal fissures in vitro. 400 sound human teeth were coated with varnish leaving occlusal and smooth surface sites unpainted and exposed for experiment. The effect of a number of acid-‘conditioning’ fluids on the enamel surface was also investigated. Window regions were exposed to one of the following solutions: phosphoric acid in the concentration range 20–70%, 50% phosphoric acid buffered with 7% zinc oxide by weight, 5 and 50% citric acid, 10% polyacrylic acid and 5% neutral and acid solutions of EDTA. Exposure times varied from 1 to 5 min. Sealants employed were Epoxylite 9070 and 9075, Elmex protector, Nuva-Seal and a material under development, TP 226. The first three materials are chemically polymerized whereas the latter two are activated by UV light. A UV fluorescent dye was added to some of the materials. Sealants were applied to previously conditioned enamel surfaces. Whole teeth and teeth slit into half were viewed with incident light. Ground sections from half of the specimens were then examined by transmitted, polarized and UV light fluorescence microscopy, and with the scanning electron microscope. Sealed teeth were also exposed to buffered lactic acid for periods ranging from 1 to 5 days. One half of the number of ground sections from all experiments was then demineralized in 10% HCl. The teeth of one child of 22 monozygotic twins were treated twice yearly with a 1-percent F–– (from SnF2) solution over a period of 9 years. When the children were 16 years old, the result was that the treated children had 37% fewer lesions than the controls. For the proximal surfaces, the reduction was 55%; for the pits and fissures, 20%; and for the free smooth surfaces, 61%. Five years after the last application, 18 pairs were examined once more. In three pairs, the controls had received full dentures. In São Paulo, Brazil, 566 children, 14–17 years old, completed a 3-year study to evaluate the anticaries effect of self-applications of acidulated phosphate fluoride (APF). There were five study groups: group A (controls) brushed with a non-fluoride prophylaxis paste and then with a placebo solution; group B brushed with the paste and then with an APF solution (0.6% F, pH 3); group C brushed with the APF solution without brushing first with the paste; group D brushed with the paste and then with an APF gel (1.23% F, pH 3); and group E brushed with the APF gel without brushing first with the paste. Fifteen brushings spaced evenly during 3 school years were made in school, supervised by lay personnel. After three years, there were reductions in incremental DMF surfaces of 26, 26, 33 and 19% in groups B, C, D and E, respectively, compared with controls. The anti-caries effect of semi-annual self-applications of an acidulated-phosphate-fluoride prophylaxis paste to the teeth of children living in non-fluoridated areas was studied. Analysis of exfoliated deciduous teeth revealed an increase in enamel fluoride concentration of 120 ppm at 5 μm in anterior teeth and 15–35 ppm in posterior teeth after each application. The acquired fluoride did not increase with further applications but instead was lost within six months. Clinical examinations revealed 21% fewer DFS in APF paste treated children than in placebo-treated children. This paper reports on some bacteriological, ultrastructural, and biochemical features of dental plaque from children living in an area of Morocco, where the caries activity is low in spite of a high sucrose intake combined with the presence of copious amounts of plaque. The most numerous groups of bacteria found were streptococci, gram-negative facultative cocci, and staphylococci or micrococci. Streptococcus mutans, S. sanguis, S. salivarius, and enterococci were present in low numbers only. Other microorganisms encountered were Lactobacillus, Actinomyces, Veillonella, Bacteroides, and Fusobacterium. Attempts were made to classify spirochetes on the basis of their ultrastructural features. The general morphology of plaque was found to compare well with previous electron microscopic descriptions of old plaque. Gram-positive as well as gram-negative cocci, rods, filaments, and occasional spirochetes were seen embedded in a matrix of varying electron density. Aggregation between similar as well as dissimilar types of bacteria was noted. Structures probably representing glycogen, lipid, or polymetaphosphate inclusions were observed in many organisms. The composition of the plaque matrix was studied by protein and polysaccharide analyses of various plaque extracts.

The relationship between serum complement-fixing antibodies against streptococcal cell walls and the incidence of caries was studied in 116 subjects. The series was divided into an ‘active caries’ (AC) group and a ‘caries-free and treated caries’ (CFTC) group, and the antibody titres were correlated with the index of decayed, missing and filled (DMF) teeth. In the CFTC group, a significant negative correlation coefficient was found with three strains of Streptococcus mutans which have been shown to be cariogenic in animals. A positive correlation was found with these streptococci in the AC group. A similar relationship was not apparent between the DMF index and the antibody titre to two non-cariogenic streptococci with either the CFTC or A
