Due to the increasing concern for transmission of transfusion-associated infectious diseases, the incidence of autologous blood donation in pregnancy has increased. The authors retrospectively identified 273 consecutive third trimester pregnant blood donors who predonated with usual monitoring precautions, save for fetal monitoring, including a requirement for a minimum Hgb/Hct of 11 g%/34%, an interval of 5–7 days between donations, and no closer than 3 days before surgery. Data elicited included donation reactions and hospital course during subsequent delivery. 268 donors donated 341 units. Seven (2.1%) had mild reactions which included pallor, diaphoresis, nausea, lightheadedness, tachypnea and tachycardia. The incidence was not different from that seen with usual homologous blood donations. In 14 patients who had external fetal monitoring there were no fetal problems save for one deceleration episode which was positionally related.

Following the establishment of an obstetric service at the authors' institution in 1982, most patients were managed satisfactorily in labor suites or on the wards, but certain critically ill patients required ICU management. The current study reviews the use of ICU facilities by the obstetric service for the period 1982–1986.

The attention of health professionals in the developing world has been chiefly centered on infants and young children, with little concern for maternal health, although the high mortality rates for mothers in rural areas and urban slums have been recognized. Indeed, of 500,000 annual maternal deaths estimated by the WHO, over 98% occur in developing countries. Maternal death rates per 100,000 live births in developing countries are of the order of 200–1000 as compared to 10–15 in the USA.

The general increase in the prevalance of cocaine use in the general population has also affected women of reproductive age during their pregnancies. Such use has been associated with deleterious effects on the developing fetus and the outcome of the pregnancy. The current study tested the hypothesis of an association between cocaine use during early pregnancy and urogenital anomalies.

Carbamapezine has been regarded as the drug of choice for women requiring anticonvulsant therapy during pregnancy, especially in view of concerns about teratogenic effects of phenytoin, valproic acid and trimetadione. Since there has been some controversy about the potential for teratogenesis in association with carbamazepine, the authors evaluated this issue retrospectively and prospectively.

Since January 1986,51 obstetric and pediatric centers in France have been involved in studies of infants born to mothers seropositive for HIV-1. All mothers entering the program were known to be seropositive and were classed according to the criteria of the CDC. Obstetricians determined the mode of delivery, breast feeding was discouraged, and infant gestational ages were assessed by both physical and laboratory methods. After birth, physical size was determined, abnormalities recorded (adenopathy, splenomegaly, neurologic signs), and infants examined every two months in the first year and every 6 months thereafter. Serologic assessment of HIV infection was done periodically after 18 months of age and infected children were also classed on the basis of CDC criteria.

Both erythromycin and tetracycline ophthalmic ointments have been said to be as effective as silver nitrate for the prevention of neonatal gonococcal conjunctivitis. These treatments have also been recommended for the management of chlamydial ophthalmia, a concept assessed by the current study.

At the authors' hospital, patients presenting for emergency cesarean section are prepared, prior to induction into anesthesia, by stomach aspiration with a wide bore nasogastric tube and the subsequent administration of sodium citrate. To assess if this approach was preferable to other techniques for combatting low gastric pH and increased gastric content volume, the authors compared three management modalities.
