Summary—The authors describe new psycho‐therapeutic method for the treatment of alcoholics. They have treated 130 patients by this “hypnopaedic” method, which is somewhere between ablation hypnosis and the hypnopedagogic verbal training methods. Of the first 50 patients, 12–30 months after the course of treatment, 36 are abstainers. The authors emphasize that they consider abstinence only to be preliminary prerequisite for the treatment; the actual aim of the treatment is the psycho‐orthopaedic correction of the personality of the alcoholics. It is possible to achieve abstinence, however, not only by aversion treatment and intolerance methods, but also by various methods, in particular, methods of suggestion.On the other hand, they also studied temperance preparations, in particular Metronidazol. They have administered 2–3 trial drinks to 70 patients after various doses and on the basis of their clinical‐pharmacological investigations they confirm the following: Metronidazol reduced the tolerance to ethylalcohol, causes in the case of therapeutic consumption milder reaction (than that of Antibuse), and in the case of daily alcohol consumption an unpleasant reaction. Apart from the noteworthy working hypotheses raised by Metronidazol, they consider the problems of sub‐acute or chronic toxicity and the possibility of changing the Metronidazol molecule, so that its “anti‐alcoholic” effect would perhaps be greater or more specific, to be very important.Résumé—Les auteurs décrivent une nouvelle méthode psycho‐thérapeutique pour le traitement des alcooliques. Ils ont traité 130 patients par cette méthode “hypnopédique” qui se situe quelque part entre l'hypnose de l'ablation et les méthodes d'entraînement verbal hypnopédagogiques. Après 12–30 mois de traitement, de 50 premiers malades, 36 sont abstinents. Les auteurs soulignent qu'ils considèrent l'abstinence seulement comme une nécessité préalable pour le traitement; le réel but du traitement étant la correction psycho‐orthopédique de la personnalité des alcooliques. Néanmoins il est possible d'atteindre l'abstinence, non seulement par un traitement d'aversion et des méthodes d'intolérance, mais aussi par des méthodes variées, spécialement par la méthode de suggestion.D'un autre côté, ils ont étudié aussi les préparations de tempérance, particulièrement le Metronidazol. Ils ont administréà 70 malades, après des doses variées de médicament. 2–3 boissons alcooliques, et basés sur les investigations clinico‐pharmacologiques, ils confirment que: le Metronidazol réduit la tolérance à l'alcool éthylique; cause, en cas de consommation thérapeutique, une réaction plus modérée que “l'Antibus”; dans le cas de consommation quotidienne d'alcool, ce produit donne une réaction déplaisante. SummaryThere were tested 1569 drinkers, among them 128 women, of different establishments of treatment, of the welfare work for alcoholic patients in big town, of the mental hospital of the University of Jena and of sanatorium. Five groups of professions were distinguished and according to them the male drinkers were examined. The great number and the changing social compositions of the drinkers in the different establishments of treatment led to certain equalization so that finally the too gross differences between groups of professions were obliterated. The different striking forms in the behaviour of the drinkers are spoken about. Also the alcoholism of women is shortly referred to. Concluding rehabilitation is considered. The chief stress is laid on the public welfare work for alcoholic patients and the detoxication cures in stationary establishments. The reception mostly takes place out of free will but then in most of the cases the difficulties of the drinkers already are so severe that the entering into hospital cannot any longer be avoided. The duration of the stationary treatment amounts to 8–12 weeks. Concluding there still follows reference to the increasing influence that the abuse of alcohol has taken on patients who are received in hospitals because of the most different psychical troubles.Résumé—Ceci est un rapport concernant l'examen de 1569 buveurs, parmi lesquels 128 femmes, internés dans différentes institutions de l'assistance publique, dans l'hôpital pour les maladies mentales appartenant à l'Université de Jena et dans un sanatorium. Cinq groupes furent classés par profession et dans ces groupes les buveurs mâles furent examinés. Le grand nombre de malades et les composantes sociales change‐antes des buveurs, dans les différentes institutions pour le traitement entra'na une certaine égalisation, ainsi que finalement les deux grosses différences entre les groupes furent effacées. Les différents aspects significatifs du comportement des buveurs sont commentés. Egalement l'alcoolisme chez les femmes est brièvement discuté. En conclusion, on commente les possibilités de réeducation. Le principal accent est mis sur l'activité de l'assistance publique des malades alcooliques, et la cure de désintoxication dans les institutions pérmanentes. L'acceptation se fait de pleingré, mais dans la majorité des cas, les difficultés des buveurs sont déjà si sévères que l'internement dans un hôpital ne peut plus être évité. La durée du traitement des internés est de 8–12 semaines. En conclusion, le rapport mentionne l'influence croissante de l'abus de l'alcool sur les personnes qui sont acceptées dans les hôpitaux à cause des troubles les plus variées.Zusammenfassung—Tests an 1569 Trinkern—128 davon Frauen—in verschiedenen Behandlungszentren wurden durchgeführt. Diese waren eine Fürsorgestelle für alkoholische Patienten in einer großen Stadt, die Nervenklinik an der Universität Jena und ein Sanatorium. Man unterschied fünf Berufsgruppen, nach denen die Untersuchung der Trinker vorgenommen wurde. Ihre große Anzahl und die Verschiedenartigkeit ihrer sozialen Stellung in den diversen Behandlungszentren führte zu einer gewissen Gleichmachung, so daß schließlich die krassen Unterschiede zwischen Berufsgruppen verschwanden. Die verschiedenen auffallenden Arten des Verhaltens der Trinker werden besprochen, der Alkoholismus der Frauen wird kurz erwähnt. Abschließend wird die Rehabilitierung erwogen.Die Bedeutung der öffentlichen Fürsorgearbeit für Alkohol‐Patienten und der Entgiftungskuren in Aufenthaltszentren wird vor allem betont. Die Aufnahme erfolgt meistens freiwillig, in der Mehrzahl der Fälle aber sind die Probleme der Trinker bereits so schwer, daß Krankenhausaufenthalt unvermeidlich wird. SummaryThis is study of two male patients addicted to alcohol, who were successfully treated by systematic desensitization. The hierarchy consists of large number of situations involving conversation with friends and strangers.It has been demonstrated in this study that the patients lose their addiction to alcohol, and may still continue socially drinking small quantities of alcohol, without developing the desire to drink to excess.The suggestion has been put forward that systematic desensitization may be preferable to aversive conditioning in the treatment of alcoholism. Some theoretical considerations are briefly discussed.Résumé—Ceci est un rapport concernant deux malades mâles, intoxiqués avec de l'alcool, qui furent traités avec succès par désensibilisation systématique. La hiérarchie consiste dans un grand nombre de situations impliquant des conversations avec des amis et des personnes étrangères. Il été démontré dans ce rapport que les malades perdent leur penchant pour l'alcool, et néanmoins peuvent continuer à boire socialement, des petites quantités d'alcool, sans développer le désir de boire excessivement. La suggéstion été avancée que la désensibilisation systématique pourrait être préférable au conditionnement répugnant du traitement de l'alcoolisme. Quelques considérations théoriques sont discutées brièvement.Zusammenfassung—Es handelt sich um einen Bericht über zwei männliche Patienten, beide alkoholsüchtig, die erfolgreich durch systematische Desensibilisierung behandelt wurden. Die Hierarchie besteht aus einer großen Anzahl von Situationen, zu denen Gespräche mit Freunden und Fremden gehören. In der vorliegenden Untersuchung wird gezeigt, daß die Patienten ihre Alkoholsüchtzgkeit verlieren und auch weiter noch bei geselligen Anlässen kleine Mengen Alkohol trinken können, ohne das Verlangen nach übermäßigem Trinken zu entwickeln. Résumé—Il est débattu que les dissemblances entre l'alcoolisme et autres intoxications citées par Pittman dans sa publication “Rush to Combine” logiquement échouent de soutenir son affirmation que les deux sphères soient traitées de la même façon. Également il y des nombreuses différences entre les patients d'une seule clinique de maladies mentales ou parmi les groupes dépendant d'autre remèdes psychoactifs ou les malades alcooliques eux‐mêmes. On arrive á la conclusion que des avantages distincts mènent á un accès compréhensif. Par exemple il y une probabilité accrue que des “intoxications mélangées” seront identifiées et traitées et que des médicaments psychoactifs seront utilisés plus judicieusement dans le traitement des intoxiqués. D'ailleurs quand les patients impliqués sont variés quant á l'utilisation des médicaments, un champ de vue plus vaste est accompli et certaines recherches qui autrement seraient négligées, sont stimulées et justifiées.Zusammenfassung—Es wird behauptet, dass die Unterschiedlichkeiten zwischen Alkoholismus und anderen Arten der Abhängigkeit von Rauschgiften, die von Pittman in seiner Abhandlung “Der Drang zur Vereinheitlichung” angeführt werden, im logischen Sinne versagen, sein Argument, dass diese beiden Bereiche nicht von ein und derselben Stelle behandelt werden sollten, zu unterstützen. Ebenso zahlreiche Unterschiede herrschen bei den Insassen ein und derselben Anstalt für Geisteskranke oder bei Gruppen, die von anderen psychoaktiven Rauschgiften abhängig sind, und selbst bei den alkoholischen Insassen. Man kam zu der Schlussfolgerung, dass durch allgemeine Angriffsmethoden ausgesprochene Vorteile erzielt werden können. So besteht z.B. eine gesteigerte Möglichkeit, dass “Mischsüchtige” identifiziert sowie behandelt und psychoaktive narkotische Drogen zur Behandlung der Abhängigkeitserscheinungen wohlüberlegter eingesetzt werden. Ausserdem wird, falls Unterschiedlichkeiten in Bezug auf die Einnahme von Rauschgiften bei den betreffenden Insassen bestehen, eine weitreichendere Perspektive erzielt und gewisse Forschungen, die andererseits vernachläs

SummarySixty‐five women with acute alcoholic psychoses were studied in an effort to investigate tinnitus, elementary visual disturbances and auditory and visual hallucinations in these states.The phenomenology of auditory and visual hallucinations was described.A substantial number of patients reported tinnitus and most of these also reported auditory hallucinations. Many patients reported simple visual disturbances and almost all of these also described visual hallucinations. There appeared to be temporal relationship concerning the onset, duration and cessation of, respectively, auditory hallucinations and tinnitus and visual hallucinations and simple visual disturbances.Disturbances in sensorium and sleep pattern regularly accompanied the onset of hallucinations. These exhibited marked improvement at the time of the disappearance of hallucinations.It was concluded that, in acute alcoholic psychotic states, repeated and monotonous auditory and visual stimulae seemed to facilitate the appearance of auditory and visual hallucinations.Résumé—Soixante‐cinq femmes souffrant de psychoses alcooliques, ont étéétudiées dans un effort de rechercher les bourdonnements d'oreille, les troubles visuels élémentaire et les hallucinations auditives et visuelles dans ces conditions. La phénoménologie des hallucicinations auditives et visuelles est décrite. Un nombre important de malades ont rapporté des bourdonnements et la plupart ont rapporté aussi des hallucinations auditives. Beaucoup de malades ont rapporté simplement des troubles visuels et presque la totalité ont décrit également des hallucinations visuelles. Il parart y avoir un rapport temporel entre le début, la durée et la cessation, respectivement entre les hallucinations auditives et les bourdonnements, et les hallucinations visuelles et les simples troubles visuels. Les troubles sensoriels et de l'aspect du sommeil accompagnaient régulièrement le début des hallucinations. Ces troubles montrèrent une amélioration marquée au moment de la disparition des hallucinations. En conclusion, dans les états psychotiques chez les alcooliques aigus, des stimuli auditifs et visuels répétés et monotones, paraissent faciliter l'apparition des hallucinations auditives et visuelles.Zusammenfassung—Fünfundsechzig Frauen mit akuten Alkoholpsychosen wurden studiert, um Ohrensausen, elementare Sehstörungen und Gehör‐ und Gesichtshalluzinationen in diesen Zuständen zu untersuchen. Die Phänomenologie dieser Halluzinationen wurde beschrieben. Eine beachtliche Anzahl der Patientinnen klagte über Ohrensausen, und die meisten unter diesen auch über Gehörhalluzinationen. Viele Patientinnen berichteten über Sehstörungen, und fast alle dieser Gruppe beschrieben auch visuelle Halluzinationen. Zwischen Einsetzen, Dauer und Aufhören von Gehörhalluzinationen und Tinnitus einerseits und Gesichtshalluzinationen und einfachen Sehstörungen andererseits schien eine zeitweilige Beziehung zu bestehen. Störungen in Sensorium‐ und Schlafverlauf begleiteten das Einsetzen der Halluzination regelmäßig. Diese zeigten eine merkliche Besserung sowie die Halluzinationen verschwanden. SummaryForcible treatment of an unwilling narcotic addict is unlikely to succeed. Treatment must maintain and reinforce motivation to be cured, which often comes about when an addict realizes that heroin has taken over his life. At preliminary interview at the patient's home it is decided whether he should be offered in‐patient or out‐patient therapy on the basis of his sex, degree of motivation, personality, age and length of time on heroin. If he is provisionally accepted as an in‐patient he attends an assessment meeting in the Unit where he is closely questioned by the patients already in treatment to determine his chances of cure. This offers an opportunity for self‐reassessment by all the patients and promotes group identification, with ego maturation which is the aim of therapy. If accepted, the new patient is given logarithmically reducing dose of heroin, with methadone support if he wishes, and is then given progressively increasing freedom until he is taking weekend leaves. His stay in the Unit is usually about 6 months. Preliminary results are encouraging.Résumé—La réussite d'un traitement énergique d'un adonné aux narcotiques, de mauvaise volonté, est improbable. Le traitement doit maintenir et renforcer le désir d'être guéri, qui survient souvent quand un adonné se rend compte que l'héroine domine sa vie. Dans un interview préliminaire au domicile du malade, on décide s'il doit être internéà l'hôpital ou traité dans un service de consultations, en tenant compte du sexe, du degré de désir d'être guéri, de la personnalité, l'âge et la longueur de la période d'intoxication à l'héroine. S'il est accepté comme malade interné, il assiste à un meeting d'évaluation dans un groupe où il est questionné attentivement par les malades déjà en traitement, pour déterminer sa chance de guérir. Cette méthode offre une opportunité de réévaluation personnelle par les autres malades et encourage l'identification de groupe avec la maturation de l'ego, qui est le but du traitement. S'il est accepté, le nouveau malade reçoit une dose de héroine de plus en plus réduite, avec l'aide du methadone, si le malade désire, et puis, progressivement on lui donne plus de liberté, jusqu'à ce qu'il reçoive la permission de sortir à la fin de la semaine. Habituellement son séjour dans le groupe est de 6 mois. Les résultats préliminaires sont encourageants.Zusammenfassung—Zwangsbehandlung eines der Behandlung abgeneigten Rauschgiftsüchtigen hat wenig Aussicht auf Erfolg. Ein Beweggrund, um die Heilung anzustreben, muß durch die Behandlung aufrechterhalten und verstärkt werden. Er ist oft dann vorhanden, wenn sich ein Süchtiger darüber klar wird, daß das Heroin sein Leben beherrscht. Bei einer ersten Unterredung im Heim des Patienten wird aufgrund verschiedener Faktoren, wie Geschlecht, Ausmaß der Beweggründe, Persönlichkeit, Alter, und Zeit seit Beginn der Heroinsüchtigkeit beschlossen, ob dem Patienten Therapie als Anstaltskranker oder ambulatorish angeboten werden soll.Wenn er provisorisch in ein Krankenhaus aufgenommen wird, nimmt er an einer Bewertungssitzung in der Abteilung teil, wo ihn bereits in Behandlung befindliche Patienten gründlich ausfragen, um die Heilungsaussichten zu bestimmen. Dies bietet eine Möglichkeit für Selbstbeurteilung aller Patienten und fördert Gruppenidentifizierung mit Egoreife, das Ziel der Therapie. Résumé—Methadone, une drogue narcotique, peut être utilisée comme un agent antinarcotique s'il est administré aux patients dans une dose constante, orale, journalière. La drogue bloquée, les intoxiqués à l'héroine ne peuvent plus l'utiliser pour obtenir l'euphorie. Ils sont dans un état normal fonctionnel et sont capables de devenir des membres responsables de leur communauté. En traitant 750 cas d'intoxiqués á l'héroine, tenaces, dans les quatres dernières années, on a eu 90% de succès.Zusammenfassung—Methadon, eine narkotische Medizin, kann als antinarkotisches Mittel verwendent werden, wenn es Patienten in gleichbleibenden, oralen, täglichen Dosen verabreicht wird. Derartig blockierte Heroinsüchtige können dieses Rauschgift nicht mehr zur Erzielung von Euphorie benützen. Résumé—Des développements récents dans le champ des intoxications avec des narcotiques ont étabil les nécessités préalables pour un accès rationnel à ces intoxications. Ceci ne dénote pas qu'un procédé définitif a étéétabli; cependant, une méthode qui peut conduire à une variété de modalités significatives en rapport avec ces différents types, peut être mis en pratique et évaluée. Puisque la structure de la société contribue elle‐même à la genèse et au maintien de l'intoxication, la résolution finale peut nécessiter des changements sociaux. La stratégie doit être une telle dans laquelle un début est fait le long de lignes qui sont practicables pour qu'on puisse procéder ainsi systématiquement au traitement et finalement empêcher les types variés d'intoxications. Les types non justiciable de techniques courantes, seront traités par le développement de nouvelles méthodes.Zusammenfassung—Kürzliche Entwicklungen auf dem Gebiete der Rauschsuchtergebenheit haben die Voraussetzung für eine rationelle Inangriffsnahme dieses Übels an den Tag gebracht. Das soil nicht besagen, dass ein definites Vorgehen festgelegt wurde sondern, dass eine Methode, die zu einer Reihe von bedeutungsvollen Ausführungsarten führen kann, welche ihrerseits für die verschiedenen Typen belangvoll sind, praktisch eingesetzt und eingeschätzt werden kann. Da unsere Gesellschaftsstruktur aus sich selbst zum Entstehen und Weiterbestehen derartiger Suchtergebenheiten beiträgt, könnte eine endgültige Schlussfolgerung zur Notwendigkeit von sozialen Umwandlungen gelangen. Die Taktik müsste so gestaltet sein, dass als Anfang, durchführbare Bahnen eingeschlagen werden, damit wir systematisch zur Behanglung und schliesslich Verhütung der verschiedenen Arten von Rauschsucht fortschreiten können.
