Child sexual abuse is a problem of major proportion that has only recently come into public focus. The numbers of reported cases of sexual mistreatment have increased, especially in the last two years. The signs and symptoms of chronic abuse may differ from those seen in acute episodes of molestation. In 75 percent of the cases, the perpetrator is known to the child. The child abuse team at Harbor UCLA Medical Center in Torrance. Calif., examines many victims of sexual abuse. The referrals come from a variety of agencies, including law enforcement and the courts in 1984. 300 children were assessed. Recognition of abnormal anal and/or genital findings is an especially frustrating aspect of these cases. This article provides information on the identification and treatment of the patient who has been sexually abused.

Nurse practitioners usually follow obese clients in outpatient settings. The behavioral approach to treating obesity has been demonstrated to help clients lose weight and maintain their weight loss. This article presents one such behavioral approach to treating obesity. Utilizing the nursing process as a framework, the components of a behavioral program are explored. Assessments of motivation, assets and limitations and target behaviors are described. Coverage of treatment planning includes contracting, problem lists and realistic commitments. Intervention techniques such as stimulus control, shaping, pairing and problem solving are presented. Evaluation using objective data gathered through monitoring is stressed. The characteristics of the practitioner which might positively or negatively affect a behavioral program for the obese client are discussed.

This study examined the attitudes of physicians working in health maintenance organizations toward the use of nurse practitioners and physician assistants. It also explored some of the underlying reasons for these attitudes: effect upon quality of care, risk of malpractice, role threat and gender bias. The setting was a health maintenance organization serving 270,000 members. The data were derived from a survey of physicians' attitudes and behavior. Physicians from internal medicine, pediatrics and obstetrics-gynecology were the study population. Internists and pediatricians had favorable attitudes toward both nurse practitioners and physician assistants. Obstetrician-gynecologists had somewhat less favorable attitudes. Physicians in all three specialties favored nurse practitioners more than physician assistants. Physicians felt that nurse practitioners were more likely to increase the quality of care and less likely to increase the risk of malpractice. Nurse practitioners were not seen as a greater role threat. Some gender bias appeared to be present, but it did not appear to constrain the use of nurse practitioners. Large, multi-specialty, prepaid group practice health maintenance organizations may be favorable settings for nurse practitioners and physician assistants to practice primary care.
