Recent reviews by Clay Smith (1992) in this journal and by W. French Anderson (1990) and A. Dusty Miller (1992) have described the technologies used for gene transfer, the potential applications of the approach, and the safety issues that require consideration. This review will have a narrower focus. It will deal specifically with current and forthcoming clinical protocols for gene transfer into hemopoietic progenitor cells, because a major goal of gene therapy for malignant and nonmalignant disease is to transfer and express genes on a long-term basis in these cells or their progeny.

Recent preclinical and clinical investigations indicate that phototherapy and photochemotherapy may have applications that go far beyond their "traditional" roles in the treatment of skin disorders, selected solid tumors, and neonatal hyperbilirubinemia. Bone marrow transplantation is one area that may benefit substantially from these new developments. This review focuses on new applications of phototherapy and photochemotherapy that pertain to the inactivation of tumor cells in autologous bone marrow grafts, the prevention and treatment of acute and chronic graft-versus-host disease, the prevention of transfusion-induced allosensitization and graft rejection, and the inactivation of pathogenic viruses and parasites in bone marrow grafts and blood products.

The kinetics of hematological recovery after autologous marrow transplantation have been studied in 70 patients with acute leukemia (38 acute nonlymphoblastic leukemia [ANNL] and 32 acute lymphoblastic leukemia [ALL]). The incidence of graft failure in this group was 3.2%, and a persistent severe thrombocytopenia was observed in 24% of the cases. Variables influencing engraftment have been studied using univariate and multivariate statistical analysis. Analysis of the entire group showed a correlation between graft colony-forming unit granulocyte-macrophage (CFU-GM) content and granulocyte recovery (p<0.001). Marrow purging was associated with a delayed engraftment (p<0.001). In ANLL patients, we found that high cummulated AraC doses before marrow cryopreservation correlated with poor granulocyte recovery after marrow infusion (p<0.002). Platelet recovery was essentially affected by age, with shorter thrombocytopenia periods in younger patients (p<0.001). Finally, excluding autotransplants with purged marrows, ALL patients showed better platelet recoveries than ANLL patients (p<0.005). These findings will be useful to evaluate the risk of delayed engraftment after autologous bone marrow transplantation (ABMT) in patients with acute leukemia.

Transplantation of bone marrow and peripheral blood stem cells was initially limited to large, research-oriented institutions. General acceptance of autologous transplantation has grown rapidly in recent years and there is now interest in the establishment of autologous transplant programs in community hospitals. However, the lack of facilities to process and cryopreserve stem cells can be a major obstacle to small transplant programs. In response to this problem, we suggest that a centralized stem cell processing facility can offer the required services to several hospitals within a metropolitan area. The establishment of processing facilities within currently existing blood banks and blood centers is a logical extension of their current roles in hematotherapy. In this paper, we describe the operation of one such centralized processing facility.

Autologous bone marrow transplantation (ABMT) appears to offer clinical benefit to leukemia patients, but the major reason for failure is relapse. This may be related to the presence of residual leukemic cells in the harvested marrow. To circumvent this problem, various procedures have been developed to purgeex vivoresidual leukemic cells from the marrow. The alkyl-lysophospholipids are a new group of anticancer drugs that target membranes as their major site of action. They are unique in that they are relatively selectively toxic to neoplastic cells and spare normal marrow stem cells and progenitor cells. The most active compound is edelfosine. Twenty-nine patients with acute leukemia in second or subsequent remission or early relapse or in first remission, either with a history of treated extra medullary relapse, or requiring more than one induction program to achieve remission, underwent ablative therapy followed by infusion of autologous marrow which had been purged by a 4-hour exposure to edelfosine prior to cryopreservation. Thirty-one percent of the patients are alive and free of leukemia for a median of 630 days (range 185-1,613). These results in this high-risk group of patients warrant further investigation.
