The occurrence of multiple sclerosis in clustered groups of cases that are often related to others in time and place has been observed on several occasions in the last 50 years. Selected clusters are here reviewed in relation to suspected sources of heavy metal (mercury, lead) poisoning as background for the analysis of the 1983–1985 “outbreak” of 30–40 cases of multiple sclerosis in Key West, Florida. Evidence is presented that the time-place clustering resulted from environmental pollution stemming from a nearby dump pile of rocky debris. The probable mechanism is discussed.

We report an unusual case of coronary-multivascular, small coronary artery embolism by atheromatous elements originating in a plague in the main right coronary artery of a 52-year-old man who died of acute thrombotic occlusion of the severely stenotic left anterior descending vessel. The embolized arteries showed the characteristic multiphasic inflammatory changes of different ages previously described in arteries occluded by this material. The myocardium supplied by the involved channels showed ischemic damage of varying ages. Both these observations indicate that the multiple emboli had been discharged at different times, rather than as a single shower. Finally, we make a plea for the use of the correct term “erosion” rather than “ulcer” for the lesion that is the source of the invasive obstructions.

A 20-year-old woman died suddenly in a hospital emergency room after presenting with nausea, vomiting, back pain, and hypertension. At autopsy, an extra-adrenal pheochromocytoma (paraganglioma) of the organs of Zuckerkandl was found, with microscopic focal myocardial necrosis similar to that described in death from adrenal pheochromocytomas. Tumors of the organs of Zuckerkandl are extremely rare; less than 100 such cases have been reported in the world's literature, and only six, including the present case, have presented as a sudden, unexpected death. The symptoms of catecholamine storm may mimic those of acute drug intoxications, leading to misdiagnosis by both clinical physicians and pathologists.

Perversion of appetite may be manifest in either qualitative (pica), quantitative (polyphagia), or combined derangements of eating. Ingested materials are capable of serving as dangerous physical agents through interference with normal cardiac and/or respiratory function. Since the mechanisms of injury are similar to those that might occur as a result of violence or serious natural disease, a thorough investigation of the history and circumstances immediately preceding the final event is warranted in addition to a complete autopsy. Three cases of asphyxia of unusual etiology are presented along with a rationale regarding the mechanisms believed to be involved. In case 1, sudden subdiaphragmatic viscus expansion with resultant lung volume displacement and impediment of venous return from the lower half of the body are believed to have been operative. In cases 2 and 3, both asphyxial loci are infraglottic. The common denominator in all of these fatalities is the physical impairment of vital air exchange as a complication of an abnormal eating pattern.

In rats, fibrinolytic activity and acidosis increased rapidly after death. Postmortem fibrinolysis and the pH, base excess (BE), and [HCO3-] levels, the higher the fibrinolytic activity. Conversely, in experiments using the vascular perfusion technique, low pH, BE, and [HCO3-] levels of the perfusate induced abundant release of plasminogen activator from the vascular wall.

Police shootings, or, when law enforcement officers shoot and kill criminal suspects, have been studied in metropolitan Dade County from 1956 to 1982. A total of 194 cases have been studied and analyzed according to age, race, sex, Spanish surname, alcohol and/or drugs detected at autopsy, and prior criminal arrest record of the victim. The reason(s) for the shooting and any agencies involved have also been noted. Finally, an appendix is included that provides guidelines for the forensic pathologist on how to approach a case.

In the spring of 1979, two water company employees were found dead at the bottom of a water-filled underground meter vault. A month-long investigation disclosed that the atmosphere of the vault had progressively been deteriorated by the growth of primitive plants present in the water and also on the walls and ceiling of the vault. When entering manholes, employees must adhere strictly to all safety rules, such as the testing of the air before entry, ventilation of the air spaces, and the presence of a standby rescue worker. The diagnosis of acute stagnant air syndrome for this type of accident is introduced herein, to alert industrial concerns and the medical professions about the existence of this occupational hazard confronting individuals who work in confined and poorly ventilated manholes.

A case of peripheral lung carcinoma clinically and anatomically simulating a malignant mesothelioma in a man who had been exposed to silicium as well as asbestos during more than 20 years of employment in industry is presented. Results of the microscopic examination of routine, stained sections were inconclusive and the importance of demonstrating mucin in the neoplastic cells by use of special staining is emphasized.
