This review describes insults to the brain which result in either an increased or decreased mass effect. These mechanisms produce certain derangements in the central nervous system, which can lead to the patient's death. Increased intracranial mass effect characterized by brain tumors may produce lethal brain-stem ischemia. On the other hand, decreased intracranial mass effect as seen in Alzheimer's disease leads to aspiration pneumonia and terminal sepsis

摘要:
This report describes the neuropathological Findings in 58 infants and children dying suddenly and unexpectedly. Utilizing historical, clinical, laboratory and pathological findings, two subgroups were distinguished: in one a cause of death was established (CODE); members of the other (more than 50% of the total sample) were victims of sudden infant death syndrome (SIDS). The importance of historical as well as pathological data in excluding SIDS is stressedIn each subgroup, both focal lesions and diffuse glial reactive hypertrophy were identified in 64% of all children below 9 months of age. These changes were not related to age group or maturation and, except for a history of perinatal asphyxia, lesions were not predictably correlated with clinical data. The brains of children dying of established cause (CODE) are not a suitable control group with which to compare those of SIDS

The pathomechanism of carbon monoxide (CO) poisoning was studied by means of cardiopathological and neuropathological investigations in experimental CO intoxication. It has been shown that besides CO-hemoglobin association, the binding of CO to cytochromes is a significant factor. The latter is thought to be responsible for the cytotoxic phenomena. Two cases of death associated with the use of lithium carbonate are described. The first one concerns a 39-year-old man who was found suddenly dead on his hospital bed. Toxicologic analysis revealed “therapeutic” levels of lithium, nitrazepam, flurazepam, and phenobarbital. The second case involves a 54-year-old woman initially treated for acute cardiac failure and hypertension, who was finally recognized as a case of lithium intoxication; she died of cardiac arrest without evident cardiac lesions and without lithium ions in serum and urine. The medicolegal aspects of both cases are discussed, with reference to the problems of diagnosis, clinical and toxicologic correlations, and the medical responsibility

This approach was required to develop techniques for constructing the impression of the fingernail used in strangulation of a victim. The illustrations show how this rare type of evidence was processed so as to match the fingernails of the suspect to the marks on the skin of the victim's neck

The author, a forensic odontologist in Liverpool, England, during his lifetime, personally investigated the cases presented which include instances of rape and battery. Between August 31 and November 9,1888, in the Whitechapel section of the East End of London, the murders of five women came to be referred to as the notorious Jack the Ripper murder cases. Each victim was a woman of the streets who was a heavy drinker and apparently heavily intoxicated when she was killed. All except one were strangled, had their throats cut, and were mutilated after they were killed. The victim who was not mutilated was strangled and saved from desecration when the killer was startled and interrupted before carrying out postmortem mutilation. The last victim was killed indoors, and all deaths occurred late in the evening or in the early morning hoursForensic science laboratories were not yet developed, so the only truly forensic examinations were made on the bodies and of the notes which the killer was alleged to have sent to the police and others. After the last crime there were no further communications with the murderer, and no one was ever charged with these crimesThis case has become the standard to which similar cases have been compared for the past century. It has been the subject for many fictionalized works written in many languages, including cinema and television productionsThis case is reviewed and evaluated as to how a similar case may be investigated using modern forensic techniques
