A man of 48 was admitted to hospital because of chronic bronchitis and difficulty in swallowing. X-ray films of the esophagus showed contrast material in the trachea but no fistula. During esophagoscopy and bronchoscopy a small opening was found on the front wall of the esophagus 22 cm from the upper incisors and 4 cm above the carina. Thoracotomy was performed with resection of the fistula. No sign of malignancy was found. According to etiology tracheoesophageal fistulas can be divided into 3 groups: 1. those caused by malignant diseases or specific infectious diseases of the esophagus; 2. those caused by trauma, and 3. congenital fistulas. In the reported case the fistula was probably congenital since no sign of malignancy was found and since the patient’s chronic bronchitis stopped entirely after the operatio

The tracheal respiratory mucosa of rats sacrificed on the second and the fourth day following experimentally induced burns of the skin was submitted to histological study. Pathological changes were found in the respiratory mucosa of all animals with hepatic and renal damage.

Fibrous dysplasia should be considered in the differential diagnosis of tumors in the face and skull. The lesion is often present already in childhood and usually is most active during the general body and bone growth period. Microscopically the lesion consists of irregularly calcified and fibrous bone trabeculae. The rate of malignant degeneration is reported to be 0.4%. The main symptom is a unilateral swelling, and the disconfiguration may be remarkable without pain or other obvious symptoms. On roentgenograms the bone at the site of the lesion appears spongy with a homogeneous, fibrous-looking pattern and a thin egg-shell type of boundary. Sometimes, instead, a dissolution of the fibrous connective tissue occurs resulting in cystformations. The treatment is surgical removal of the diseased tissue; radical, mutilating operations are, however, not indicated. Twelve cases are reported. The lesion occurred in 7 males and 5 females, the maxilla was involved in 8 cases, the mandible in 2 cases, the frontal bone in 1 and the temporal bone in 1. The average duration of the illness was 8 years and the average age at onset around 17 years. Pronounced disconfiguration was seen in most cases, and extensive surgery was necessary in 8 of the patients. Malignant degeneration was not seen.

Three cases with swellings at the angulus mandibulae have been examined. From clinical, roentgenological, electromyographical, and microscopical findings it was shown that the swellings arose from masseter hypertrophy caused by hyperfunction in connection with bruxism.

Results of bacteriological analyses carried out in 480 chronic ears showed that initially, 401 ears (83.5%) were infected; the most frequent bacteria were Ps. aeruginosa and St. aureus (22.5 and 21.3%). Local preoperative treatment reduced the number of infected ears to 48.2%. Specimens from the mastoid antrum and/or cells were positive in 50% of cholesteatoma ears and 20% of chronic ostitic ears. Bacteria were cultured even in 11% in dry or culture-negative ears. Ps. aeruginosa strains were most sensitive to streptomycin (69%) and next to colistin (31%). St. aureus was best sensitive to streptomycin (90%) and chloramphenicol (72%). Against Proteus strains, streptomycin was the most effective (82%) followed next by chloramphenicol (70%). Postoperative wound infection appeared in 11 ears (2.3%) and was most often due to St. aureus. Prolonged middle ear and ear canal infection continued postoperatively in 37 ears (7.7%), the main agent being Ps. aeruginosa.

It is demonstrated that local diagnosis of the hearing impairment and evaluating of the degenerating process in the central nervous system in Tay-Sachs disease may be possible by means of various kinds of audiometric methods applicable to young children, such as cerebral evoked response audiometry using a computer as well as startle response, auropalpebral reflex (APR) and cochleopupillary reflex (CPR) audiometries. Thresholds of startle response, APR, CPR during sleep and cerebral evoked potentials in response to sound stimuli were measured in three patients with Tay-Sachs disease. The thresholds of the startle response, APR and CPR were found to be comparable to those in normal newborn infants and normal young children, whereas no auditory cerebral evoked responses were detected by an average response computer even at the maximum intensity except for a case with the threshold of 80 dB. Since the startle response as well as APR and CPR may be integrated at the level of the brain stem and the cerebral evoked response may arise from the cerebral cortex over a wide area, it is suggested that the lack of the auditory cerebral evoked response probably reflects a predominant degeneration of the higher supranuclear auditory pathways.

The deviation of the eyes induced by vestibular stimuli is defined as the distance in degrees from the mid-position of the eyes to the position to which the eyes return in the fast phase. At an intensive angular stimulus the eyes are found to lead the head and to such an extent that the whole nystagmus beat can be displaced in the direction of the fast phase. The deviation of the eyes in the fast direction seems to be closely related to the deflection of the cupula and subsides exponentially in the postacceleratory period in about the same way as does the speed of the slow component. The eyes, however, reach their mid-position before the nystagmus disappears, indicating that a certain degree of intensity in the stimulus is needed to make the eyes deviate in the direction of the fast phase.
