Desmoid tumor in the maxillary sinus is a rare condition. The present case occurred in a 3 7-year-old male complaining of left nasal obstruction and dull pain in the nape of the neck. The clinical picture, pathology, diagnosis and prognosis are discussed and the literature is reviewed.

To elucidate a limit to the efficiency of the upper airway mucosa as an air conditioner, the temperatures of the inspiratory air and mucosa were measured in the cervical trachea. Both of them were affected only minimally by change of atmospheric air temperature during resting nose breathing, but were affected greatly by change of mode of breathing. During hyperventilation through the mouth, when the atmospheric air temperature was 1 °C, a temperature difference of 9 °C was noted between inspiratory air in the cervical trachea and body temperature, together with a mucosal temperature fall by 1.86 ± 0.61 °C. Benign pedunculated tumours of the hypopharynx are very rare. Only a few cases of hypopharyngeal fibrolipomas have been reported. This paper adds 2 more cases with characteristic clinical features. We also present a case of recurrent fibromatous tumours of the hypopharynx with malignant change after 20 years. Malignant transformation of an originally benign hypopharyngeal tumour is extremely unusual. The clinical and pathological aspects of pedunculated benign tumours of the hypopharynx are discussed. Symptoms and signs are few and uncharacteristic. The diagnosis is difficult and often delayed. There is a risk of suffocation by laryngeal obstruction by the tumour. When the accurate diagnosis is established, the treatment of choice should be a lateral pharyngotomy with complete removal of the tumour. Long term follow-up is recommended.

Ignition or combustion of the endotracheal tube during CO2 laser microlaryngo surgery may cause fatal lung disorders. We undertook an experiment on ignition and combustion of endotracheal tubes by CO2 laser beam and came to the following conclusions: (a) the silicon endotracheal tube was nonflammable; (b) the spiral endotracheal tube was also nonflammable; (c) a pulse wave was recommended and 3-second intervals were necessary; (d) four sheets of moist gauze were necessary to prevent cuff trouble, and (e) it was advisable to set the O2 concentration below 25 % when using the silicon endotracheal tube.

Although prostaglandins (PGs), especially PGE2 and PGF2α, are thought to be associated with inflammation, there have been no papers dealing with the synthesis of PGE2 and PGF2α in human tonsillar lymphocytes. The features of PG synthesis in lymphocytes were investigated by incubating cells with radiolabelled precursors. Synthesis of PGE2 was greater than that of PGF2α in tonsillar lymphocytes. PGE2 was the predominant product in the tonsillar lymphocytes of the recent infection group. There was no significant differene between T and B lymphocytes in the ratio of PGE2 and PGF2α formed. A significant increase in PGE2 formed with bradykinin was found in the tonsillar hypertrophy group. A serious anaerobic sampling and cultivation technique was used in order to obtain representative samples from 45 middle ear effusions in 30 patients with longstanding secretory otitis media (SOM). Despite the fact that an accurate sterilizing procedure was performed, 16 % disclosed contaminated samples from the ear canal and tympanic membrane. Among the remaining samples, middle ear bacteria were found in 30 % of which Staphylococcus epidermidis and Haemophilus influenzae contributed to the major part. Anaerobes were only occasionally found. The amount of bacteria found was sparse in all cases. It can be concluded that the inflammatory process in SOM is not maintained by anaerobic bacteria within the middle ear cavity.

This paper studies the changes in speech discrimination following stapes surgery in patients with advanced otosclerosis. Two thirds of these patients showed a postoperative improvement in the discrimination score with obvious correlation with closure of bone-air gap. The results were slightly worse in comparison to those obtained on otosclerotic patients with good bone conduction. In advanced otosclerosis even a minimal impairment in bone conduction and/or an incomplete closure of the gap interfere with discrimination.

The protective effect of fosfomycin against aminoglycoside (dibekacin)-induced ototoxicity was studied in rats. Rats were injected with 100 or 50 mg/kg of dibekacin with or without 500 mg/kg of fosfomycin for 60 or 120 consecutive days. Inner ear damage appeared to be more reduced histopathologically in animals given both dibekacin and fosfomycin than in animals given dibekacin alone. Similarly, renal damage appeared to be reduced histopathologically and functionally by the combined administration of dibekacin and fosfomycin. The mechanism of reduced ototoxicity may be as follows: fosfomycin inhibits the accumulation of dibekacin in the kidney, and reduces its concentration in the kidney and serum. Consequently, the amounts of dibekacin reaching the inner ear are decreased, and ototoxicity is reduced.

In order to examine the frequency of a target wave and induced eye movements we designed and developed a new target wave (continuously changing sinusoidal wave). This target wave consisted of 20 sinusoidal waves from 0.1 to 2.0 Hz with 10° amplitude (peak-to-peak amplitude is 20°). Several parameters were calculated quantitatively with a microcomputer in normal subjects, and the following conclusions were obtained: The amplitude gain (peak-to-peak amplitude of eye movements/peak-to-peak amplitude of the target wave) and the retinal error velocity were appropriate parameters for representing the input and output of smooth pursuit.
