This is a report of cochleo-saccular degeneration found in temporal bones from a patient who had suffered from slowly progressive and total sensorineural deafness which had an inherited origin. At age 8, this patient began to complain of hearing loss, and by age 10 she was totally deaf. The patient was 1 of 3 female siblings who have suffered from an exactly identical progressive disease: deafness, absent gastric motility, small bowel diverticulitis and ulceration, and sensory neuropathy. The temporal bone pathology found in this case was the degenerative change in the cochlear duct and sacculus. No pathology was found in the utriculus and semicircular canals.

The superior vestibular nerve was studied histologically in 25 patients who underwent removal of inferior vestibular nerve schwannoma. In most cases, the nerve fibers were structurally normal but the endoneurial space showed various degrees of capillary stasis and/or extravasation of red blood cells. Increased fibrosis of the endoneurial space was seen in about two-thirds of the cases; it was mild in all except two cases. The endoneurial space appeared edematic in about one-third of the cases. Wallerian degeneration of individual fibers was occasionally observed in most cases and was severe in the two cases with moderate or severe endoneurial fibrosis. In about one-third of the cases there were large numbers of myelinated nerve fibers with thinned myelin sheaths. The observed structural changes are compatible with those seen in human and experimentally induced chronic compressive or entrapment neuropathies. Thus, early symptoms of eighth-nerve tumors might develop due to direct compression of the eighth-nerve trunk within the internal auditory canal.

Head and eye movements induced by vestibular stimuli (rotational velocity steps) were studied under several experimental conditions in squirrel monkeys. Monkeys showed prominent head nystagmus under a free head condition. Slow phase gaze velocity (SPGV) was calculated by summing the slow phase speed of coupled head and eye nystagmus. A statistical test showed no significant difference between slow phase eye velocities in a fixed head versus SPGV in a free head condition. This indicates that an accurate negative feedback control of eye movements is performed by the vestibular system when sensing active head movements. Bilateral lateral semicircular canal block eliminated these responses in subjects without amphetamine injection. However, in amphetamine-injected animals, 14–21% of preoperative response could be found even after this operation. In order to study the effects of head rotations on optokinetic responses, we compared optokinetic nystagmus on gaze recordings between pursuits with the head fixed, with fixed-head oscillations and with the head free. Although subjects could perceptionally well pursue stripes even at 120 and 150°/sec in case of pursuits with the head free, while fixed-head oscillations inhibited trackings, we could not find a significant difference of pursuit gaze speed among the different conditions. The result indicates that pursuit gaze speed is decided exclusively by the stripe speed, irrespective of head movements, and that the most important factor to decide perception is the pursuit angle of the tracked stripes rather than the pursuit speed

The present investigation of a retrospective series of patients with tongue cancers aimed at assessing the results of various treatment principles. We suggest that surgery must be taken into account particularly when large primary tumors as well as palpable regional nodes are found. Because of the unpredictable metastasis behavior, even in cases of small primary tumors without clinical evidence of regional spread, the potential risk of regional microdeposits must always be suspected. Therefore, irradiation of the neck field with fractionated doses of 5,000 rad, alone or in combination with surgery should be considered in all patients with tongue cancers.
