Summary:Nonconvulsive seizures were divided into the two major classes used in the International Classification of Seizures (partial and generalized). Of the patients with nonconvulsive seizures reported, four had complex partial seizures and two had generalized absences. An analysis of the simultaneous recordings of EEG activity and the behavior of patients with nonconvulsive seizures on closed circuit television (CCTV) led to improved recognition and management of the seizures by providing information essential for making proper diagnoses and classification. Some anatomical and physiological factors are offered to explain the limited ability of scalp EEG to register epileptogenic spike activity occurring at the cortex.

Summary:An 81‐year‐old woman with rotatory seizures and a right frontal lobe malignant brain tumor is described. This association has not been previously reported, to our knowledge. Evidence for a possible dopamine receptor‐mediated pathway of body rotation during rotatory seizures is discussed.RÉSUMÉLes auteurs décrivent le cas D'une femme ágée de 81 ans qui a des crises rotatoires et une tumeur cérébrale maligne du lobe frontal droit. Summary:The symptomatology, electroencephalographic and other correlates, development, and genetics of a new mutant in mice for spontaneous seizures are described. This recessive mutant is designated “‘spontaneous seizures” and is assigned the gene symbolsps.Just at or after puberty, 25% of thesps/spshomozygotes show behavioral arrest and spontaneous generalized convulsions. The behavioral arrest is associated with 1 ‐2/s high‐voltage spikes in the neocortex and the generalized convulsions are associated with paroxysmal activity in the neocortex. The effects of this mutant are compared with those of others for reflex or spontaneous seizures in mice.RÉSUMÉCe travail décrit les formes D'épilepsie, leur expression électroencéphalographique, leur développement, ainsi que les caractéristiques génétiques D'un nouveau mutant chez la souris qui présente des crises spontanées. Summary:Common behavioral alterations associated with epilepsy include increased interest in philosophical and religious concerns, increased and extensive writing of a cosmic or philosophical nature, changes in sexual behavior, and aggressiveness. Psychological stress, the effects of anticonvulsant therapy, and the actual occurrence of seizures or convulsions can be ruled out as possible causes of the syndrome. It is speculated that these behavioral alterations are the result of an intermittent spike focus in the temporal lobe that leads to an alteration in the responsiveness of the limbic system. Thus, there is a heightened emotional response to many stimuli as well as a decrease in sexual responsiveness. In an effort to discover the cause of the high incidence of sexual alterations, abnormalities in response to luteinizing hormone‐releasing hormone (LHRH) were found in a group of patients with partial complex seizures, some of whom had no overt sexual dysfunction and had never received anticonvulsant therap

Summary:Homozygoustotteringmice (tg, autosomal recessive) exhibit frequent spontaneous “absence” seizures accompanied by bilaterally synchronous spike‐and‐wave or polyspike electrocorticographic discharges. In adulttotteringmice, the antiepileptic effects of a single dose of ethosuximide, diazepam, phenobarbital, or phenytoin were assessed using continuous electrocorticographic recording to monitor seizure incidence. The dose chosen for each drug was selected to correspond to an effective antiepileptic dose in standard murine models. Ethosuximide, 150 mg/kg, diazepam, 1.4 mg/kg, and phenobarbital, 25 mg/kg were effective against absence seizures. In contrast, phenytoin at 5, 10, 30, or 60 mg/kg produced no significant reduction in the incidence of absence seizures. These results suggest that absence seizures in thetotteringmutant may represent a relatively specific pharmacological model for the identification of drugs effective for clinical absence epilepsy, and emphasize the potential value of this new model for the study of fundamental mechanisms of absence of epilepsy and the actions of antiepileptic drugs.RÉSUMÉLes souris homozygotes“chancelantes”(tg.autosome récessif) présentent de fréquentes crises spontanées de type “absence,” accompagnées de décharges électrocorticographiques bilaterales synchrones de pointe‐ondes ou polypointes. Chez les souris“chancelantes”adultes, les effets antiépileptiques D'une dose unique D'ethosuximide, diazepam, phénobarbital ou phénytoine, ont été testés sur un enregistrement électrocorticographique continu permettant de déterminer L';incidence des crises. Pour chaque drogue, la dose était choisie pour correspondre à une dose antiépileptique efficace dans les modèles murins standards. Les doses de 150 mg/kg D'ethosuximide, 1,4 mg/kg de diazepam et 25 mg/kg de phénobarbital étaient efficaces contre les absences. A L';inverse, la phenytoine à 5, 10, 30 ou 60 mg/kg ne produisait pas de réduction significative dans la survenue des absences. Ces résultats suggèrent que les absences chez la souris mutante“chancelante”peuvent représenter un modèle pharmacologique relativement spécifique pour L';identification des drogues efficaces dans L';épilepsie cliniquement caractérisée par des absences. Ils soulignent également la valeur potentielle de ce nouveau modèle pour étudter les mécamsmes fondamentaux de L';épilepsie à type D'absences et les effets des drogues antiépileptiques.RESUMENLos ratonestumbaleanteshomocigotes (tg, autosómico recesivo) presentan frecuentes “ausencias” espontáneas acompañadas de descargas electrocorticográficas bilaterales y síncronas de punta‐onda o polipuntas. Usando registros encefalográficos continuos para monitorizar la incidencia de los ataques se han determinado los efectos antiepilépticos en los ratonestambaleantesde dosis únicas de etosuximida, diazepan, fenobarbital y fenitoina. La dosis elegida para cada droga se correspondía con la dosis antiepiléptica eficaz en modelos “murina” standard. La etosuximida (150 mg/kg), el diazepan (1.4 mg/kg) y el fenobarbital (25 mg/kg) fueron eficaces contra los ataques de ausencias. For el contrario la fenitoina a dosis de 5, 10, 30 o 60 mg/kg no produjo una reduccion significativa de la incidencia de las ausencias. Estos resultados sugieren que los ataques de ausencias en el mutantetambaleantepuede representar un modelo farmacológico relativamente específico para iden‐tificar drogas eficaces contra las ausencias y destacan el valor potencial de este nuevo modelo pura el estudio de los mecanismos fundamentales de la epilepsyía de ausencias y de la acción de las medicaciones antiepilépticas.ZUSAMMENFASSUNGHomozygote Tottering‐Mäuse (tg, autosomal recessiv) zeigen häufige spontane “Absencen,” von bilateral synchronen spike‐wave oder polyspike‐wave Ent‐ladungen elektrocortikographisch begleitet. Bei er‐wachsenen Tottering‐Mäusen wurde die antiepilep‐tische Wirkung einzelner Dosen von Ethosuximid. Diazepam, Phenobarbital oder Phenytoin mit Hilfe einer kontinuierlichen elektrocorticographischen Ab‐leitung zur Bestimmung der Anfallshäufigkeit verifiziert. Die Dosierung für jedes Medikament wurde in Übereinstimmung mit den effektiven antiepileptischen Dosen des standardisierten Mäusemodells gewählt. Ethosuximid (150 mg/kg). Diazepam (1.4 mg/kg) und Phenobarbital (25 mg/kg) waren gegen Absencen wirksam. Im Gegensatz hierzu zeigte Phenytoin in einer Dosierung von 5, 10, 30, 60 mg/kg keine deutliche Verminderung der Häufigkeit von Absencen. Diese Anfälle lassen vermuten. Daß Absencen bei der Tot‐teringmutante ein relativ spezifisches pharmako‐logisches Modell zur Identifizierung von Medikamen‐ten darstellen, die bei der klinischen Absence‐Epilepsie wirksam sind. Summary:Because nonepileptic disorders may cause episodic and paroxysmal symptoms that resemble epilepsy, these conditions and syndromes must be considered in the differential diagnosis or when antiepileptic drugs are ineffective. Gastroesophageal reflux usually presents as recurrent vomiting in infants and young children. A small subgroup of infants develop apnea and cyanosis accompanied by posturing of the limbs, deviation of the eyes, or opisthotonos, which leads to the incorrect diagnosis of epilepsy. Breath‐holding spells and pallid infantile syncope, common in infants and young children, may also be mistaken for epileptic attacks. The parasomnias, including pavor nocturnus and somnambulism, are frequently mistaken for epilepsy, since these nocturnal episodes are paroxysmal in nature, may be associated with automatic behavioral mannerisms, and tend to be recurrent. Migraine is especially difficult to differentiate from epilepsy because its manifestations, particularly in children, are so diverse. Summary:EEG monitoring was performed on a group of ten children, aged 3 days to 13 years, during admission to the Paediatric Clinic at the University of Bologna for seizures. Diazepam was injected by rectal route as an anticonvulsant, using a dose of 0.5‐1 mg/kg in the first nine children, whereas an intravenous bolus was administered to the last child. The main aim of this study was to document the time taken for the drug to reach the brain and to modify the electrical activity. We observed significant EEG changes between 1 and 9.30 min and, in particular, the appearance of fast activity over one or both hemispheres after rectal diazepam. The EEG results confirm the clinical efficacy of rectal diazepam.RÉSUMÉDans un group de dix enfants, agées de trois jours à treize ans, observés dans le laboratoire de EEG de la Clinique de Pédiatric (Université de Bologne), un contrôle EEG pendant la crise convulsive a été exécuté. Diazepam a été introduit par voie rectale, dans le but D'arrêter la crise (0.5– 1 mg/kg) chez neuf enfants, et par voie intraveineuse comme bolus (1.5 mg) chez un enfant pris comme contrôle. Etant donnée L';efficacité D'une telle voie thérapeutique, on a churchéà demontrer la rapidité D'action du médicament, même utilisé par voie rectale, pour rejoindre le cortex et modifier L';activitéélectrique du cerveau. Dans la majorité des cas, avec un retard de 1 min à 9 min 30 s, on a observé L';apparition D'une activité rapide diffuse sur les deux hémisphères et en relation avec L';introduction du medicament. La vitesse des modifications bioélectriques cérébrales semble confirmer L';efficacité clinique de L';utilisation du diazepam par voie rectale.RESUMENDurante su ingreso en la Clínica Pediátrica para ataques (Universidad de Bolonia) se ha monitorizado el EEG en un grupo de diez niños de tres días a trece años de edad. Como anticonvulsivante se administeró diazepan por vía rectal (0.5‐1 mg/kg) a los primeros nueve niños mientras que se inyectó un bolo intravenoso al último niño. El propósito fundamental de este estudio consistió en documentar el tiempo que tarda la droga en alcanzar el cerebro y modificar la actividad eléctrica. Se observaron cambios significativos en el EEG entre 1 y 9,30 minutos y, en particular la aparición de actividad rápida en uno o ambos hemisferios tras la administración de diazepan rectal. Los resultados electroencefalográficos confirman la eficacia clínica del diazepan rectal.ZUSAMMENFASSUNGEtne EEgraphische Monitorisierung wurde in einer Gruppe von zehn Kindem im Alter zwischen drei Tagen und dreizehn Jahren durchgeführt, die wegen Krämpfe in die Kinderklinik der Universität Bologna eingewiesen worden waren. Bei neun Kindern wurde als anticonvulsives Mittel Diazepam rektal in einer Dosis von 0,5‐1 mg/kg Kp‐Gew. verwendet, bei einem Kinde wurde das Mittel intravenös gegeben. Hauptziel dieser Untersuchung war, die für das rektal gegebene Mittel notwendige Zeit zur Erreichung des Gehirns und zur Beeinflussung der elektrischen Aktivität zu ermitteln. Ein bedeutender Einfluss auf das EEgraphische Bild wurde in einer Zeit zwischen 1 und 9 Minuten nach rektaler Verabreichung vom Diazepam beobachtet, mit Auftreten von einer Schnellen Aktivität in einem oder beiden Hemisphären. Summary:Clinical course and long‐term seizure prognosis were studied in 155 patients with complex‐partial seizures during a follow‐up of 10.1 ± 1 (SD) years. In 79% of the patients generalized tonic‐clonic seizures were recorded, mostly within the first 3 years of epilepsy but occurring as late as 20 years after the onset of epilepsy. Seizure control was defined as complete absence of all seizures, including auras, for a minimum of 2 years. Seizure control occurred in 20 of 32 patients (63%) with complex‐partial seizures only and in 76 of 123 patients (62%) with complex‐partial seizures and generalized tonic‐clonic seizures. The onset of the epilepsy with generalized tonic‐clonic seizures or complex‐partial seizures did not influence the therapeutic outcome despite differences in their natural history. A family history of epilepsy and other generalized seizures (e.g., absence) were more frequent in patients with generalized tonic‐clonic seizures at the onset of epilepsy. Seizure control was significantly lower (44%) in patients with a history of a maximum frequency of one or more generalized tonic‐clonic seizures per month when compared to patients (79%) with a total of less than six generalized tonic‐clonic seizures (p<0.05). The frequency of generalized tonic‐clonic seizures is of predictive value for the seizure prognosis of patients with complex‐partial seizures.RÉSUMÉInvolution clinique et le pronostic à long terme des crises ont étéétudiés chez 155 patients qui avaient des crises partielles à sémiologie complexe pendant une periode de 10,1 ± 1 (SD) ans. Chez 79% des patients des crises géneralisées tonico‐c/oniques ont été enregistrées, la plupart dans les 3 premierés années de L';épilepsie, mais pouvant sur‐venir jusqu'a 20 ans apres le début. Le contrôle des crises a été défini comme L';absence compléte de toute crise, y compris les auras, pendant au moins 2 ans. Le controle des crises a été obtenu chez 20 des 32 patients (63%) qui avaient seulement des crises partielles complexes, et chez 76 des 123 patients (62%) qui avaient à la fois des crises partielles complexes et des crises tonicocloniques généralisées. Le fait que L';epilepsie ait débuté par des crises tonico‐cloniques généralisées ou par des crises partielles complexes n'avait pas dinfluence sur le résultat thérapeutique, malgré la différence qui existe dans leurs histoires naturelles. En el 79% de los enfermos se registraron ataques gene‐ralizados tónico‐clónicos y, la mayor parte, en los primeros tres años de la epilepsia aunque podian ocurrir incluso 20 años despues del comienzo. El control de los ataques se definió como la ausencia total de cualquier tipo de crisis, incluyendo las auras, durante un minimo de dos años. Este control se observÓ en 20 de 32 pacientes (63%) con solo crisis parciales complejas y en 76 de los 123 (62%) con crisis parciales complejas asociadas a crisis generalizadas tónico‐clónicas. El comienzo de la epilepsfa con crisis tónico‐clónicas generalizadas o ataques parciales complejos no in‐fluencio el resultado terapeutico a pesar de las diferen‐cias de sus historias naturales. La historia familiar de epilepsfa y otros tipos de crisis generalizadas (por ej. ausencias) se encontraron mas frecuentemente en enfermos con crisis tónico‐clónicas generalizadas al comienzo de la epilepsfa. El control de los ataques fué significativamente más bajo (44%) en pacientes con una historia de máxima frecuencia de una o más crisis generalizadas tónico‐clónicas al mes compareándolo con enfermos (79%) con un total de menos de seis crisis generalizadas tónico‐clónicas (p<0.05). Bei 79% der Patienten wurden generalisierte, tonisch‐klonische Anfalle registriert, meistens ereigneten sie sich in‐nerhalb der ersten 3 Jahre der Epilepsiekrankheit; sie wurden jedoch auch 20 Jahre nach Epilepsiebeginn beobachtet. Als Anfallskontrolle definierten wir eine Freiheit von alien Anfällen einschlierßlich isolierten Auren für mindestens 2 Jahre. Summary:Eliminating seizures should be the first goal of therapy for non‐convulsive epilepsies, but preventing seizures, i.e., guarding against head injuries and immunizing against agents that attack the nervous system, is the second goal. An accurate diagnosis of seizure type helps ensure that the appropriate medication for that particular form of epilepsy will be prescribed. Drug decisions should also be based on the risk:benefit ratio to the individual patient, and drug interactions should be considered when more than one drug is required. Frequent monitoring of drug serum levels is necessary in the case of multiple drug therapy or until seizures are controlled. Ethosuximide is considered the drug of choice in absence seizures, but valproic acid is equally effective. Although effective in controlling absence seizures, clonazepam is not favored in this indication because of a high incidence of side effects and the development of tolerance. Atonic seizures are generally refractory to treatment, but valproate, clonazepam, and occasionally carbamazepine represent the drugs of choice in management. Phenytoin continues to be a very popular drug for most types of seizures, but carbamazepine, used adjunctively until recently, is effective as monotherapy for the control of partial seizures, particularly those of the complex partial variet
