Summary:The key elements in the development of epileptogenesis appear to be the capacity of membranes in some (pacemaker) neurons to develop intrinsic burst discharges, the presence of disinhibition, and the proper excitatory synaptic circuitry. It is likely that the relative role of each of these processes will differ at different sites in the central nervous system which are prone to epileptogenesis. Synchronization of neuronal populations is a vital element in the development of focal discharge and a variety of mechanisms, including those dependent upon excitatory postsynaptic potentials, and other interactions are possible. Pathological processes may alter some or all of these regulatory mechanisms. However, different pathological entities presumably produce epileptogenesis through different combinations of pathogenetic mechanisms.

Summary:There is insufficient clinical information about carbamazepine and other newer anticonvulsants from tropical African countries because high costs and limited availability have restricted the use of these drugs in the economically underdeveloped countries. The present communication reports the observed effects of carbamazepine in nine Nigerian children whose epileptic seizures had been intractable to various combinations of previously available anticonvulsants. In seven of these children, intractable seizures were associated with moderate to severe cognitive deficits and behavioral disturbances. Following institution of carbamazepine therapy, complete seizure control was achieved in four children, and significant reduction in seizure frequency occurred in another four. In three children complete seizure control was maintained with carbamazepine alone after withdrawal of all other medications. Among the seven children with cognitive and behavioral problems, improved seizure control was accompanied by significant psychotropic gains. No serious side effects occurred in any of the children during follow‐up periods ranging from 2 to 7 years. Because of the favorable anticonvulsant and psychotropic efficacy of carbamazepine in the present study, coupled with the lack of serious side effects, it is recommended that the drug deserves wider use in African children with seizure disorders.RÉSUMÉOn manque d'informations sur l'utilisation de la carbamazepine et d'autres médicaments antiépileptiques récents en provenance des pays d'Afrique tropicale, en raison du coût de ces médicaments et de la difficultéà se les procurer, qui en a limitea l'utilisation dans les pays économiquement sous‐développés. La présente communication rapporte l'effet de la carbamazepine chez neuf enfants Nieegriens dont les crises n'avaient pas été contrôlées jusque là par diverses combinaisons d'autres médicaments. Chez sept des neuft enfants, les crises étaient associées à des déficits cognitifs, modérés à sévères, et à des troubles du comportement. Le traitment par la carbamazepine a amené le contrôle complet des crises chez auatre enfants, et une readuction significative de leur nombre chez quatre autres. Chez 3 enfants, lecontrôle complet des crises s'est maintenu après la suppression de tous les médicaments autres que la carbamazepine. Sur les 7 enfants ayant des troubles cognitifs et comportementaux, I'amélioration du contrôle des crises s'est accompagnée d'une amélioraton significative de l'état psychique. Aucun effet toxique sériekux n'a été observé pendant une période d'administration de 2 à 7 ans. En raison de l'actiion anticonvulsivante et psychotrope favorable de la carbamazepine, et de l'absence d'effets toxiques sérieux, it est recommandé que la carbaxazepine soit plus largement utilisée chez les enfants africains atteints d'épilepsie.RÉSUMÉLa insuficiente informatión clínica acerca de la carbamazepina y otras medicaciones anticonvulsivas, má nuevas, que se deriva de paéses topicales africanos es debida al alto costo y a la escasa disponibilidad de estas medicaciones que han limitado su uso en los países económicamente en vías de desarrollo. Esta comunicación informa acerca de los efectos observados tras la administración de carbamazepnia a nueve nińos nigerianos que habían tenido ataques epilépticos sin respuesta terapeútica a varias combinaciones de anticonvulsivos, previamente utilizadas. En siete de los nueve niños los ataques resistentes a la terapeútica estaban asociados a moderados o saveros defectos intelectuales y trastornos del comportamiento. A patir de la iniciació de la terapia con carbamazepina se consiguió un total control en cuatro niños, a la vez que se observaba una reducción significative de la frecuencia de los ataques en cuatro casos adicionales. En tres niños el control completo se mantuvo con carbamazepina como tratamiento único. una vez que se interrupie ron las medicaciones previas. Los siete niños con problemas de comportamiento y de inteligencia la mejoria de los ataques se acompañó de ganacias psicotrópicas significativas. Summary:Epileptic mechanisms in the brain are subject to long‐duration, time‐ordered neuromodulatory processes controlled by endogenous oscillators which are responsible for appropriately phased modulation of various normal physiological processes, including the 24‐h sleep/wakefulness cycle and the ultradian 100‐min cycle of rapid eye movement/non‐rapid eye movement sleep. Both focal and generalized types of epileptiform activity in humans are subject to biorhythmic modulation, and the various modulation patterns observed are in accord with a model which explains these patterns as a consequence of the interaction of two endogenous modulatory processes: one with a period of about 24 h, the other with a period of about 100 min. Differences in the phase angle between the two cyclic processes, determined by time of sleep onset, explain the various modulatory patterns observed. The mechanisms involved in the genesis and elaboration of electrical epileptiform activity in animal models are examined in relation to known processes involved in the physiology of sleep, and compared with data derived from long‐term studies of the time distribution of epileptic events in humans. In infantile spasms, clinical seizure activity and the ictal and interictal EEG patterns in relationship to the phases of the sleep cycle, the significant defects in the quality and quantity of sleep in this disorder, and the changes that take place in all of these when seizures are abolished by effective treatment, suggest that pontine mechanisms responsible for the sleep cycle may be involved in the elaboration of infantile spasms and hyp

Summary:We report data from four patients with unilateral epileptiform status activity within different structures of the temporal lobe, recorded during stereoelectroencephalographic presurgical evaluation. The ictal clinical symptoms accompanying neocortical and mesiobasal limbic discharges (two patients with complex partial status epilepticus) consisted of various psychosensory and vegetative signs, which can be understood on the basis of the spatiotemporal analysis of the discharges. The other two patients with circumscribed long‐lasting mesiobasal limbic epileptiform activity represent unilateral pure limbic status epilepticus and were characterized by a marked behavioral syndrome (with stickiness, aggressivity, etc.) and a monomorphic gustatory aura continua, respectively. The latter patient, with left hippocampal discharges of nearly continuous epileptiform character, was also assessed with tachistoscopic tasks. Performance revealed cognitive impairment only in the epileptically discharging hemisphere and in dependence on the quality of the EEG pattern. After unilateral selective amygdalohippocampectomy, the two patients with limbic status epilepticus were seizure free and had markedly improved behavior and cognitive functions. Thus, patients with nonconvulsive status epilepticus represent an ideal model, although rare, to correlate behavior alterations and brain dysfunction.RÉSUMÉDes données sont rapportées sur quatre patients présentant une activitéépileptique continue unilatérale dans différentes structures du lobe temporal, enregistrée pendant l'étude stéréoencéphalographique pré‐chirurgicale. Les symptômes cliniques critiques qui accompagnent les déchargesnéo‐corticales et limbigues mesio‐basales(2 patients ayant un état de mal de crises partielles complexes), consistent en des signes pscyhosensorield et végétatifs qui peuvent étre expliqués sur la base de l'analyse spatio‐temporelle des décharges. Les deux autres patients ayant des décharges eépleptiques mésio‐basales limbiques localisées de longue durée représentent un état de malunilatéral purement limbiquecaractérisÉ ches l'un par un syndrome comportemental important (avec viscosité, agressivité etc…) et chezl'autre par une aura continue gustative monomorphe. Le dernier patient, ayant des décharges hippocmpiques gauches pratiquement continues a été examiné en ce qui concerne ses performances tachistoscopiques. L'étude de ces performances montre une atteinte des fonctions cognitives concernant seculement l'hémisphère où siègent les décharges et dépendant de la qualité du pattern EEG. Après une amygdalohippocampectomie sélective unilatérale, les 2 sujets ayantun état de mal limbique n'ont plus eu de crises et ont eu une amélioration marqueée de leur comportement et de leurs fonctions cognitives. Ainsi les sujects présentat un état de mal épileptique non convulsif représentent un modèle idéal, quoique rare, pour étudier les corrélations entre les altérations du comportement et la dysfonction cérébrale.ZUSAMMENFASSUNGBei vier Patienten, die wegen pharmako‐reistenter komplexpartieller anfalle im Hinblick auf eine chirugische Epilepsietherapie stereo‐elektroenzephalographisch abgeklärt wurden, fand sich in verschiedenen Strukturen des Temporallappens epileptische Entaladungstätigkeit im Sinne eines Status epilepticus. Das klinische Erscheinungsbild bei den zwei Patienten mit neokortikalen und limbischen Entaladungen (komplex partieller Status epilepticus) war unterschiedlichund beinhaltete verschiedene psychosensorische und vegetative Symptome, die sich in Kenntnis des räumlich‐zeitlichen Entaladungsmusters verstehen lassen. Bei den anderen zwei Patienten fand sich ein unilateraler, rein mediobasal limbischer Status epilepticus. Das einemal vergesellschaftent mit eindrücklichen Verhaltensstörungen (Umständlichkeit, Episoden erhöhter Reizbarkeit, usw), das anderemal mit einer einförmigen gustatorischen Aura continua. Bei letzterer Patientin mit linksseitigem, hippokampalen Status epilepticus zeigten tachistoskopische Untersuchungen hemisphärenspezifische kognitive Defizite in abhÄngigkeit vom Muster der epileptischen Entladung. Nach unilateraler Amygdala‐Hippokampektomie sind beide Patienten, die den limbischen Status epilepticus aufwiesen, anfallsfrei und zeigen eine deutliche Verbesserung der Verhaltensstörungen. Summary:Homocysteine thiolactone causes convulsions when administered to animals, and has recently been reported to have excitatory effects on neurons in the central nervous system. Glutamic acid diethyl ester (GDEE) has previously been found to be an effective antagonist of the central excitation induced by homocysteine and is thought to be a selective antagonist of the quisqualate‐sensitive excitatory amino‐acid‐receptor site. If an interaction of homocysteine with the quisqualate‐sensitive receptor site is responsible for its convulsive properties, GDEE might also block the induction of seizures by homocysteine. GDEE in a dosage of 4 mmol/kg almost completely blocked homocysteine‐induced seizures in mice; smaller dosages had no effect or only slight inhibitory effects. Glutamic acid dimethyl ester (GDME) and glutamic acid γ‐methyl ester (GMME) also partially blocked homocysteine‐induced seizures, but monosodium glutamate and glutamic acid γ‐monoethyl ester (GMEE) had only a slight effect. None of the glutamate esters inhibited seizures induced by pentylenetetrazole. It is therefore suggested that certain types of seizures involve the quisqualic acid excitatory amino‐acid‐receptor site. Homocysteine‐induced seizures may serve as a model of seizures of this type, and GDEE, GDME, and GMME may be effective antagonists of such seizures.RÉSUMÉLorsqu'on l'administre à l'animal la thiolactone homocystéine provoque des convulsions et il a été récemment rapporté qu'elle a des effects excitants sur les neurones du système nerveux central. Le diethyl ester de l'acide glutamique (GDEE) s'est montré un antagoniste efficace de l'excitation centrale indquite par l'homocysteine el l'on pense qu'il a un effet antagoniste sélectif du site récepteur de l'amino‐acide excitateur sensible au quisqualate. Si une interaction de l'homocysteine avec le site récepteur sensible au quisqualate est responsable de ses propriétés anticonvulsives, le GDEE pourrait aussi bloquer l'induction de crises par l'homocystéine. Le GDEE, à une dose de 4 mMole/kg, a presque complètement bloqué les crises induites par l'homocysteine chez les souris. Des doses inférieures n'ont eu que peu ou pas d'effet inhibiteur. El ester dietilico del ácido glutámico (GDEE) ha sido consiserado como un antagonista eficaz de la excitación central inducida por la homocisteína y se piensa que es un antagonista selectivo del receptor excitatorio de aminoácidos, sensible al quiscualato. Si la interacción de la homocisteína con el receptor sensible al quiscualato es responsable de las propiedades convulsivas, el GDEE podria también bloqucar la inducción de los ataques por homocisteína. El GDEE en dosis de 4 mMole/kg bloqueó casi totalmente los ataques inducidos por la homocisteína en ratones. Summary:Seizures frequently manifest autonomic dysfunction clinically, and seizure discharges commonly spread into and involve autonomic pathways. These associations are direct and simple in some instances, and the result of multiple indirect and complex relationships in others. Effects of epileptic discharge on the autonomic nervous system are mediated through the cortical, limbic, and hypothalamic systems. Some significant consequences of altered autonomic function include convulsive apnea, abnormal sexual function, and potentially fatal effects on the cardiovascular system.

Summary:The intellectual performance of 350 patients with seizures was examined. Seizures were classified according to the revised International Classification of Epileptic Seizures as partial (simple or complex), generalized, or partial secondarily generalized. Performance on the Wechsler Intelligence subtests served to measure intellectual ability. Patients with partial seizures performed better on Digit Span, Digit Symbol, Block Design, and Object Assembly than those with either generalized or partial secondarily generalized seizures. These subtests are functionally associated with abilities such as attention, visuospatial problem‐solving, and sequencing ability.RÉSUMÉLes performances intellectuelles de 350 patients épileptiques ont étéévaluées dans ce travail. Les crises ont été classées selon la nouvelle Classification Internationale des Crises Epileptiques en crises partielles (simples ou complexes), généralisées ou partielles secondairement généralisées. Les résultats aux subtests de l'échelle d'intelligence de Wechsler ont servi de mesure des captacités intellectuelles. Les patients ayant des crises partielles avaient de meilleurs résultats à l'empan verbal (span), au code, aux cubes et aux assemblages do'objects que les patients avec crises généralsées ou avec crises partielles secondairement généralisées. Ces subtests sont liés aux fonctions d'attention. de structuration visuospatiale et d'habileté dans les tâches séquentielles.RESUMENEn este estudio se ha examinado la capacidad intelectual de 350 pacientes con ataques. Summary:Many factors associated with hormone function have an impact on the course of epilepsy. Patients with epilepsy may have disturbances in sexual function such as anovulatory cycles in women and decreased libido and potency in men. Data indicate seizures, especially those arising in the limbic system, may influence the hypothalamic pituitary axis. Antiepileptic drugs also influence sexual function through direct brain effects as well as through induced changes in pharmacokinetics of the sex steroid hormones. Pregnancy has been reported to be a time of increased seizures; however, this has often been associated with low drug levels, for reasons that include inadequate drug dose, possible changes in pharmacokinetics, and noncompliance. Some evidence suggests that hormones affect seizure frequency. Changes in seizures during the menstrual cycle (catamenial epilepsy) have been found in some women: seizures were fewer during the luteal phase but increased when progesterone levels declined. Some improvement in seizure frequency has been shown in pilot studies using medroxyprogesterone acetate, a synthetic progesterone. Current concepts of the interrelationship among epilepsy, sex hormones, and antiepileptic drugs are discussed.

Summary:Various parameters of anion and cation transport were measured in the cerebral cortex of neonatal (3‐day‐old) and adult rats following acute and chronic treatment with phenytoin (PHT). Acutely, PHT significantly inhibited the enzyme Na+, K+‐ATPase in both neonatal and adult rats. This effect was accompanied by a significant increase in cerebral cortical Na+content and a decrease in K+content only in neonatal animals. Chronic treatment (two and four times a day for 7 days) of adult rats with PHT significantly reduced Na+content without affecting whole homogenate Na+, K+‐ATPase activity. The activity of this enzyme was markedly increased in the myelin‐ (glial product) and slightly decreased in the synaptosomal‐ (neuronal) fractions following chronic (four times a day for 7 days) PHT treatment. These results suggest that PHT differentially affects the two forms (neuronal and glial) of the enzyme Na+, K+‐ATPase. The possible relevance of this hypothesis in relationship to the anticonvulsant and excitatory properties of PHT is discussed. Chronic (two and four times a day for 7 days) PHT treatment increased both DNA content and activity of the glial marker enzyme carbonic anhydrase. Activity of the mitochondrial enzyme HCO3‐‐ATPase was also increased following chronic PHT treatment. These two enzymes are intimately involved in the regulation of HCO3‐‐Cl‐transport across glial cell and mitochondrial membranes, and these results suggest that PHT is able to affect beneficially glial regulatory processes. The ability to enhance glial regulation of anions and cations in extracellular fluid provides new and important insights into the mechanism of the anticonvulsant action of PHT.RÉSUMÉDifférents paramètres du trasportanionique et cationique ont été mesurés dans le cortex cérébral de rats nouveaux‐nés (3 jours) et adultes après traitement aigu et chronique par la PHT. En traitement aigu la PHT inhibait de fa¸on significtive l'enzyme Na+, K+‐AtPase chez les rats adultes et nouveaux‐nés. Cet effet s'accompagnait d'une augmentation significative du contenu cortical en sodium (Na+) chez les animaux nouveaux‐nés seulement. En traitement chronique (2 fois/jour et 4 fois/jour pendant 7 jours) ches les rats adultes la PHT réduisait de fa¸on significative le Na+sans affecter l'ensemble de l'activité Na+,K+‐ATPase. L'activité de cet enzyme était nettement augmentée dans la fraction myéline (produit glial) et légèrement augmentée dans la fraction synaptosomique (neuronale) après traitement chronique par la PHT (4 fois/jour pendant 7 jours). Ces résultats suggèrent que la PHT affecte de fa¸on différente les 2 formes (gliale et neuronale) de l'enzyme Na‐, K+‐ATPase, Les auteurs discutent l'importance possible de cette hpothèse par rapport aux properiétés anticonvulsives et excitatrices de la PHT. Le traitement chronique par la PHT (2 fois/jour et 4 fois/jour pendant 7 jours) augmentait à la fois le contenu en DNA et l'activité du marqueur glial anhydrase carbonique. II y avait aussi une augmentation de l'activité de l'enyzme mitochondrial HCO3‐‐ATPase.
