SUMMARYThe literature comprises many reports on haematological complications during anticonvulsant medication. Systematic haematological surveillance of epileptic patients is considered necessary to prevent these complications. This paper discusses ways and means to ensure maximum efficacy of this surveillance in clinics without a specialized haematological laboratory. A description is also given of methods suitable for supplementing and extending the routine examinations.RÉSUMÉLa littérature comporte de nombreuses observations de complications hématologiques au cours des traitements anticonvulsivants. SUMMARYUrine of 130 children and adults referred to the EEG laboratory of a mental health clinic were “screened” for inborn metabolic errors by means of a battery of simple, inexpensive tests. Hyperaminoaciduria, mellituria or both were detected in 13 (10%) of the group, and results were confirmed by more elaborate examinations.There were 113 patients with abnormal EEG's, of whom 12 also excreted abnormalities in the urine. There were 84 mentally retarded patients, of whom 11 had abnormal urines. The greatest incidence of abnormal urines occurred in young, retarded children with minor motor attacks, clinically; generalized multiple spike and slow wave discharges electrographically. No patients with classical petit mal attacks without retardation had confirmed, reproducible abnormal urine tests, although in several there was a transient increase in cystine. There was no correlation of focal EEG abnormality and degree or type of urinary abnormality.Seizure control was markedly improved in 2 patients when the abnormal urine metabolites were detected and the appropriate dietary regime established. As expected, because of the age of the patients, there was no effect on retardation.On the basis of this study, it is mandatory that “screening” tests for abnormal urinary metabolites be performed routinely on all retarded children with minor motor attacks clinically and paroxysmal discharges electrographically. This can be done initially with sufficient accuracy by means of the battery of tests described.RÉSUMÉL'urine de 130 enfants et adultes adressés au Laboratoire d'E.E.G. d'une Clinique de Santé Mentale, a été analysée avec des moyens simples et économiques, pour mettre en évidence une éventuelle erreur métabolique. Une hyperaminoacidurie, une méliturie, ou les deux ensemble, ont été détectées chez 13 sujets (10% des cas) et confirmées par des examens plus précis.Des 113 sujets qui présentaient un E.E.G. anormal et des 84 qui souffraient d'un retard mental, 12 et 11 respectivement présentaient des urines anormales. Ces dernières provenaient principalement des sujets les plus jeunes, qui présentaient un retard mental, des crises épileptiques mineures et des décharges de polypointe‐ondes lentes, sur l'E.E.G. II n'existait pas, au contraire, d'anomalies des urines (sauf une augmentation transitoire éventuelle de la cystine) chez les sujets souffrant d'un petit mal clinique, sans retard mental. Il n'y avait aucune relation entre les anomalies E.E.G. focalisées et le degré ou le type des anomalies urinaires.Le contrôle thérapeutique des crises fut considérablement amélioré chez deux sujets après que l'erreur métabolique fut reconnue grâce à l'examen urinaire et un régime diététique établi de façon appropriée. SUMMARY1EEG data, together with clinical findings, are presented in two groups of epileptics of widely differing age: 177 patients older than 51 years, and 177 patients between the ages of 21 and 40.2Evidence of organic etiology of seizures (vascular disorders, brain tumors and degenerative diseases) was more frequently seen in the older group; seizures of unknown etiology were more common in the younger group.3A greater incidence of focal EEG abnormality was found in older adults; a greater incidence of generalized abnormality was observed in the younger group. SUMMARYOvert physical aggressiveness was found to be present in 36 out of 100 consecutive temporal lobe epileptics selected for a unilateral anterior temporal lobectomy. Most of the aggressive patients were young boys with an early age of onset of epilepsy and a left (dominant) temporal lobe focus. The role of impaired learning in the pathogenesis of aggression was discussed and the need for a more intensive study of the aggressive outburst itself—preferably with electrical methods—was stressed.RÉSUMÉUne agressivité manifeste a été rencontrée chez 36% des sujets souffrant d'une épilepsie du lobe temporal qui avaient été sélectionnés en vue d'une lobectomíe temporale antérieure. SUMMARY1The effect of GABA on strychnine potentials recorded from the gyrus ectosylvius of cats anaesthetised with chloralose—urethane was examined.2It was shown that in the presence of GABA, these potentials, as recorded at the surface of the cortex, were reversed in polarity, this finding applying to both components of the wave. If the cortex was explored with a depth electrode, it was found that GABA applied to the surface of the cortex did not affect the strychnine potentials.3Trichloracetic acid, 10%, applied to the surface of the cortex, produced the same effect on the strychnine potentials as GABA.4The most likely explanation of the influence of GABA seems to be that it blocks both excitatory and inhibitory,i.e.depolarizing and hyperpolarizing, synapses on the surface of the cortex, while neuronal activity persists in the deeper layers.5The origin of the second, surface‐positive, component of the strychnine potential is also discussed.RÉSUMÉ1L'effet du GABA sur les potentiels strychniniques du gyrus ecto‐sylvius du chat anesthésié par le chloralose‐uréthane est étudié.2Il est démontré que, en présence de GABA, les deux composants du potentiel strychninique enregistréà la surface du cortex sont inversés en polarité. SUMMARYActivations were performed by means of natrium‐evipan (Bayer). Small doses (0.10–0.20 g) of evipan were administered intravenously. The drug was injected slowly, concomitantly with continuous recording of the EEG. In all the cases solutions of 0.05 g evipan/ml normal saline or distilled water were injected. The rate of administration was, on the average, 1 ml solution every 10 sec. Evipan‐induced EEG activation was achieved following standard bipolar recordings with 3 min of hyperventilation. Eighty‐three patients and 4 normal controls underwent evipan EEG activation. The patients were studied in the following groups: 41 with epileptic seizures ascertained clinically; 14 with non‐epileptic paroxysmal manifestations (8 with seizures indefinite in clinical aspect, 3 with syncopal seizures, 1 with migrainous attacks and 2 with paroxysmal vertigo (Menière)); 21 neurotic cases; 7 focal cerebral lesions (2 tumors and 5 cerebrovascular accidents). The bioelectric changes produced by administration of small doses of evipan were not accompanied by disagreeable subjective sensations nor by sleep. The main duration of an activation is 20 min at the maximum. After this time interval the patient can leave the laboratory room without any risk whatsoever.The results obtained show that evipan constantly induces bioelectric changes such as: diffuse synchronization or resynchronization of the background activity, increase in tracing amplitude and appearance of focal anomalies or diffuse, lesional or irritative anomalies. The latter were particularly conspicuous in patients clinically showing epileptic seizures. It is worth noting that the increase in amplitude occurring in epileptics with evipan exceeds 2–3 times the amplitude of the initial EEG tracings. The increase is very early in epileptics following very small doses of evipan and lasts longer than with the other categories of patients. This high voltage amplitude of bioelectric tracings was interpreted as a functional reorganization phenomenon, diffuse or restricted to particular epileptogenic circuits and systems of the brain. The amplitude increase was regarded as pathognomonic for epilepsy, as it only occurred in patients with clinical epilepsy and was constantly associated with comitial, diffuse or focal anomalies.On the basis of data obtained, the amplitude increase was estimated as an electro‐encephalographic equivalent of the convulsive predisposition of the brain.In a great number of cases evipan activation induces electroencephalographically a delimitation of lesional or epileptogenic foci. The setting off of foci is accounted for by the absence of bioelectric resynchronization induced by evipan at the level of the damaged cerebral areas.RÉSUMÉDes activations ont été effectuées avec l'Évipan sodique (Bayer) en solution de 0.05 g/ml dans du sérum physiologique. De petites doses du produit (0.10 à 0.20 g) ont été utilisées en injection intra‐veineuse lente (1 ml de solution en 10 secondes) pendant un enregistrement continu de l'E.E.G.Quatre‐vingt trois malades et 4 témoins normaux ont étéétudiés dont on avait préalablement enregistré l'E.E.G. standard comportant une épreuve de l'hyperpnée de 3 minutes. Les malades se groupaient de la façon suivante: 41 épileptiques; 14 sujets souffrant de crises non épileptiques (crises de nature indéterminée, 8 cas; syncopes, 3 cas; migraine, 1 cas; vertiges paroxystiques, 2 cas); 21 névrotiques; 7 sujets souffrant de lésions cérébrales en foyer (tumeur, 2 cas; foyer vasculaire, 5 cas).L'activation de ces sujets par l'Evipan sodique s'est effectuée en 20 minutes au maximum et ne s'est accompagnée d'aucune sensation subjective désagréable. Cette activation s'exprime par une synchronisation ou une resynchronisation diffuse de l'activité fondamentale de l'E.E.G., phénomène qui est particulièrement précoce, important et durable chez les épileptiques dont l'amplitude des tracés est doublée ou même triplée. Chez les sujets souffrant de crises épileptiques focalisées et chez ceux porteurs d'une lésion focalisée non épileptogène, l'activation fait apparaître des anomalies E.E.G. focales de type irritatif ou non irritatif ou augmente l'amplitude dc telles anomalies lorsqu'elles préexistaient.Pareilles modifications des tracés sont interprétées par les auteurs comme la manifestation d'un phénomène de réorganisation fonctionnelle, diffuse ou localisée, de certains circuits et systèmes épileptogènes du cerveau; elles sont considérées comme pathognomoniques de l'épilepsie et représenteraient un

SUMMARYFourty‐one patients resident in Chalfont Epileptic Colony were treated with Trin‐uride.Three patients were withdrawn from the trial as a result of rapid deterioration in the epilepsy of one, confusion and disorientation in another and incapacitating ataxia in the third.There was an improvement in the major epilepsy of 49% of cases; in 29% there was no change; in 17% there was a deterioration and in 5% the drug was withdrawn due to severe side effects.The most frequently observed change in behaviour was that of aggression.The maximum dosage of Trinuride, the raised serum alkaline phosphatase and leucopenia observed in some patients, are discussed.RÉSUMÉQuarante et un sujets résidents à la Colonie pour Épileptiques de Chalfont ont été traités par la Trinuride.Trois ont été soustraits à l'effet du produit à cause d'une aggravation rapide des crises dans un cas, d'une confusion avec désorientation dans un autre, et d'une ataxie dans le troisième.Il y eut une amélioration de l'épilepsie majeure dans 49% des cas, aucun effet appréciable dans 29%, aggravation dans 17% et soustraction du produit pour des effets secondaires sévères dans 5%.La modification de comportement la plus souvent observée fut l'aggressivité.Les auteurs discutent le dosage maximum de Trinuride, ainsi que l'élévation de la phosphatase alcaline sérique et la leucopénie obs

SUMMARYIn the years 1949, 1954, 1959 and 1964 theAmerican Institute of Public Opinionobtained answers to questions about epilepsy from representative members of the adult population throughout the United States. During this 15‐year period knowledge of epilepsy was found in 92–95 % of the people interviewed. When those familiar with epilepsy were asked if they would object to their children playing with epileptics, 57% answered no in 1949, 68% in 1954, 67% in 1959 and 77% in 1964. When asked if they thought epilepsy was a form of insanity, 59% said no in 1949, 68% in 1954, 74% in 1959 and 79% in 1964. When asked if epileptics should be employed, 45% said yes in 1949, 60% in 1954, 75% in 1959, and 82% in 1964. The adverse responses were proportionately reduced. In each of the four surveys, the most favorable opinion was among the better educated, better employed, younger and urban members of the population. The states in the Eastern, Midwestern and Western regions showed less prejudice than those in the Southern region. These measures of public opinion indicate a distinct improvement in general attitude toward epilepsy.
