SUMMARYIf the patterns of seizure disorders in parents and offspring could be assessed accurately, some insight might be gained into the relative role of genetic and environmental factors in the development of convulsive disorders. In this study, 908 children born in a Rochester, Minnesota hospital from 1922 through 1972 whose mother or father had a verified and classified diagnosis of seizure have been followed from birth for evidence of any convulsive episode or seizure disorder. The observed numbers of various types of convulsions in the offspring are compared to the expected number based on local age‐specific incidence rates. The outstanding finding was that a higher than expected number of children whose mothers had epilepsy also suffered from seizures (epilepsy or febrile convulsions), whereas no such increase was detected among the children of affected fathers.RÉSUMÉSi l'on pouvait évaluer avec précision le type d'épilepsie chez les parents et les descendants, on pourrait obtenir des renseignements sur le rôle par les facteurs génétiques et l'environnement dans le développement de Pépilepsie. On a essayé de préciser l'existence de tout épisode convulsif ou paroxystique chez 908 enfants suivis dès la naissance, nés dans un Hôpital à Rochester, Minnesota, entre 1922 et 1972, et dont le père ou la mère ont eu une forme d'épilepsie bien caractérisée. Le nombre des différentes convulsions observées chez les enfants avec un parent épileptique, a été comparéà celui qu'on peut s'attendre à observer dans une population du même âge. Se han observado, desde su nacimiento, 908 niños nacidos en un hospital de Rochester, Minnesota, desde 1.922 a 1.972. cuyos padres o madres habían sido diagnosticados de desórdenes convulsivos. Se han comparado el número y los diversos tipos de convulsiones que ocurrieron en los niños con el número teórico de ataques que deberían haber tenido según los índices de frecuencia para su edad y región geográfica. SUMMARYIt has been suggested that patients with epilepsy and particularly those on long‐term anticonvulsant medication may have a lower than expected risk of Ischemic heart disease. The records of a cohort of patients with epilepsy in Rochester, Minnesota were reviewed to ascertain their rates of occurrence of Ischemic heart disease. The results did not show any relative decrease in the incidence or mortality rates due to Ischemic heart disease among men or women with epilepsy. The numbers of Ischemic heart disease incidence and mortality cases were 25 and 15, respectively, relative to corresponding expected values of 15.0 and 15.7 new and fatal events. The use of anticonvulsant medications did not appear to influence the rates of Ischemic heart disease among the patients with epilepsy. Subgroups of the epilepsy patients, by etiology and types of epilepsy, were not found to account for a disproportionate share of the Ischemic heart disease. The survivorship of epilepsy patients after the initial manifestations of Ischemic heart disease was comparable to that expected among all Ischemic heart disease patient

SUMMARYThe results of surgical treatment of intractable epilepsy seizures in a group of patients investigated with the intracarotid amobarbital‐pentylenetetrazol EEG test were analyzed. This diagnostic test, originally designed to distinguish a mechanism of primary from that of secondary bilateral synchrony in the EEG, was also used in a small group of patients with independent bilateral temporal epileptiform spiking. The test has been carried out in 99 patients of which 34 were operated on. Not all the operated patients fulfilled the test criteria for secondary bilateral synchrony.The follow‐up studies demonstrate that when the test results point to a lateralized predominance of the epileptic mechanisms, regardless of whether or not the test pattern fulfills the criteria of secondary bilateral synchrony, a circumscribed excision of epileptogenic brain tissue under cortical electro‐encephalographic control can be expected to produce a reasonably satisfactory reduction of seizure incidence in about half the patients. Asymmetries of the arterial supply to the cerebral hemispheres, however, must be taken into account. The validity of the test results is greatly impaired when the carotid artery on the side presumed to be the more epileptogenic on the basis of the test results supplies not only the ipsilateral but also part of the contralateral hemisphere.The lack of any important difference in the follow‐up results between patients with a test pattern of secondary bilateral synchrony and those in which the test results indicated a diffuse or multifocal epileptogenic condition with a lateralized predominance suggests that the classical concept of secondary bilateral synchrony must be revised. This condition probably represents only a particular variety of generalized epilepsy (often caused by widespread brain lesions) in which a localized area of cortex has assumed a dominant pacemaker role in the precipitation of generalized epileptic discharges. There is probably no sharp dividing line between this form of generalized epilepsy with secondary bilateral synchrony and others in which a wide range of asymmetry of the epileptogenic process can be demon

SUMMARYA patient with hyperthyroidism is described who developed grand mal seizures when anti‐thyroid medication was withdrawn. Pyramidal signs were also present. SUMMARYUtilizing an alumina‐gel epileptic monkey model, with instrumentation for continuous monitoring of all overt, spontaneous motor seizures, the efficacy of pharmacologic prophylactic treatment of posttraumatic epilepsy was explored. The alumina‐gel model provides a relatively standardized brain trauma from monkey to monkey, resulting in virtually complete assurance that all animals will manifest, in time, electrical and clinical seizures if not treated. Thirteen rhesus monkeys were divided into two groups of 8 drug‐treated and 5 placebo animals, respectively. Administration of diphenylhydantoin and phenobarbital in a combined regimen commenced within 48 hr of the alumina‐gel injections. After 1 year the monkeys were withdrawn from either their drugs or placebo and followed for a subsequent 4 month period. The data for the first 12‐month period indicate that anticonvulsant treatment of potentially epileptic monkeys decreased both the frequency and severity of seizures they would have had without treatment. All animals manifested an electrical focus and overt seizures, but the drug monkeys had only partial seizures whereas the placebo monkeys exhibited secondarily generalized tonic‐clonic seizures. The follow‐up, no‐treatment data of 4 months are reported in the following paper.RÉSUMÉL'efficacité du traitement pharmacologique prophylactique de l'épilepsie prosttraumatique a étéétudié chez le singe rendu épileptique au moyen de gel d'alumine, avec un appareillage enregistrant en continu toutes les crises motrices spontanees évidentes. Le gel d'alumine permet un traumatisme cérebral relativement standardisé et donne l'assurance virtuelle que tous les animaux manifesteront tôt ou tard des crises electriques et cliniques en l'absence de traitement. Treize singes rhesus ont été répartis en deux groupes, un de huit singes traités et un de cinq singes placebo. L'administration simultanée du DPH et phenobarbital a commencé pendant les 48 heures qui ont suivi les injections de gel d'alumine. L'administration des médicaments ou des placebo a cessé au bout d'un an et les singes ont été suivis pendant les quatre mois suivants. Les résultats des douze premiers mois montrent que le traitement des singes potentiellement épileptiques a diminuéà la fois la fréquence et la sévérité des crises qu'ils auraient eu sans traitement. Trece monos de la especie rhesus fueron divididos en dos grupos de 8 tratados con drogas y 5 tratados con pastillas de azúcar, respectivamente. Das “alumina‐gel” Modell sorgt für ein relativ standardisiertes Trauma von Affen zu Affen mit dem praktisch fast sicheren Resultat, dass alle Tiere mit der Zeit elektrische und klinische Anfälle erleiden, wenn sie nicht behandelt werden. Dreizehn Rhesusäffchen wurden in zwei Gruppen von acht drogenbehandelten und fünf Placebo‐Tieren eingeteilt. Eingebung von Diphenylhydantoin und Phenobarbital in einer zusammengefassten Diät, begann innerhalb von 48 Stunden nach den Einspirtzungen mit “alumina‐gel.” Nach einem Jahr wurden die Affen ihren Drogen oder Placebos entzogen und für eine darauffolgende Periode von vier Monaten beobachtet. SUMMARYThis study, a 4‐month follow‐up period of a 12‐month treatment study by the present authors, was concerned with the permanent effects of treatment with diphenylhydantoin and phenobarbital in the alumina‐gel monkey model. Whereas the 8 drug animals during withdrawal increased their seizure frequency, duration, and severity, those 4 animals having received 120 mg/kg/day DPH in weeks 6–12 had one‐half the number of seizures of the 4 placebo monkeys in the follow‐up period. The other 4 drug animals who had continued to receive 60 mg/kg/day DPH during those weeks had two to four times the number of seizures of the placebo group during posttreatment. (All drug monkeys received 80 mg/kg/day of DPH from weeks 13–52 and 6 mg/kg/day of phenobarbital throughout the 12‐month treatment period.) The results reaffirm the problems of drug withdrawal and the importance of altering seizure mechanisms with sufficiently high doses of efficacious anticonvulsants rather than merely treating epileptic manifestations at lower doses.RÉSUMÉCette étude reporte les effets permanents du. departement par le phénobarbital et la diphénylhydantoine chez le singe rendu épileptique au moyen de gel d'alumine, Les effets ont été observés pendant une période de 4 mois consécutive à un traitement de 12 mois entrepris par les mêmes auteurs. Après retrait du médicament, la fréquence, la longueur et la sévérité des crises augmentèrent chez les huit singes traités. Mais les 4 animaux ayant recu 120 mg/kg/jour de DPH pendant les semaines 6–12 n'eurent que la moitié des crises présentées par les 4 animaux placebo, tandis que les autres 4 animaux qui recurent 60 mg/kg/jour pendant ces semaines ont eu 2 à 4 fois plus de crises que le groupe placebo après retrait du médicament (Tous les singes traités recurent 80 mg/kg/jour de DPH pendant les semaines 13–52 et 6 mg/kg/jour de phénobarbital tout au long des 12 mois de traitement). Mientras que en los 8 animales drogados y retirados de las drogas la frecuencia, severidad y duración de los ataques aumentó durante el periodo del retiro de éstas. En aquellos cuatro animales que recibieron 120 mg/kg/dia DPA en 6 a 12 semanas tuvieron la mitad de ataques que los cuatro monos que no recibieron nada en el periodo continuado. Los otros cuatro animales drogados que continuaron recibiendo 60 mg/kg/dia DPH durante esas semanas, tuvieron de 2 a 4 veces el numero de ataques del grupo de monos en las pastillas de azucar despues del tratamiento. (Todos los monos drogados recibieron 80 mg/kg/dia de DPH a partir de las semanas 13–52 y 6 mg/kg/dia de phenobarbital a traves del periodo de 12 meses de tratamiento. Es händelte sich um einen Zeitraum von vier Monaten weiterverfolgender Beobachtungen von einem insgesamt zwölfmonatigem Behandlungsexperiment der gegenwärtigen Autoren. Während die 8 Drogentiere die Schwere, Dauer und Häufigkeit der Anfälle innerhalb der Entwöhnungszeitspanne erhöht sahen, hatten jene vier Tiere, die 120 mg/kg/pro Tag DPH während der 6, bis 12, Woche erhalten hatten, die Hälfte der Anfälle der vier Placebotiere in der darauffolgenden Beobachtungsperiode. Die anderen 4 Drogentiere, die während jener Wochen weiterhin 60 mg/kg/täglich erhalten hatten, erfuhren zwei bis viermal die Anzahl von Anfällen der Placebo‐Gruppe während der Nachbehandlung. SUMMARYNinety‐one patients operated on for focal epilepsy between 1952 and 1973 have been followed regularly. Nontumoral lesions were found in 68 patients, and tumors in 23. Of the 50 patients without tumor who have been followed for more than 2 years, 30% were seizure‐free, 50% improved, while only 20% did not benefit from the operation. There was no operative mortality. Ventricular asymmetry in the pneumoencephalogram was associated with a better prognosis than when there was no or symmetrical enlargement. The time between the onset of seizures and the operation did not affect the prognosis, but early operation is important to achieve better and quicker rehabilitation. There was a positive correlation between the postoperative ECoG and EEG and the clinical results.RESUMENSe han observado con regularidad 91 enfermos operados de epilepsía focal. Se encontraron tumores en 23 pacientes y lesiones no tumorales en 68. De los 50 enfermos sin tumor y observados durante más de 2 años, 30% estaban sin ataques, 50% mejorados y solamente un 20% no se benefició de la intervención. No hubo mortalidad operatoria. La asimetría ventricular en la pneumoencefalografía se acompañó de mejor pronóstico que la simetría. El tiempo transcurrido entre el comienzo de los ataques y la intervención no alteró el pronóstico pero la operación precoz es importante para conseguir una mas rápida y mejor rehabilitación. Se encontró una correlación positiva entre el Electrocorticograma, el Electroencefalograma y los resultados clínicos.ZUSAMMENFASSUNGEinundneunzig zwischen 1952 und 1973 wegen fokaler Krampfanfälle operierte Patienten wurden regelmäβig nachuntersucht. Bei 68 Patienten waren nicht‐tumorbedingte Läsionen, bei 23 waren Tumoren gefunden worden. Unter jenen 50 Patienten, bei denen kein Tumor gefunden wurde und die länger als 2 Jahre kontrolliert wurden, waren 30% anfallsfrei, 50% gebessert und nur 20% ohne positiven Operationseffekt. Es gab keine operationsbedingten Todesfälle. Ventriculäre Assymetrie im PEG war mit einer besseren Prognose korreliert als symmetrische Befunde in der Luftfällung. Die Prognose wurde nicht beeinflusst durch das Zeitintervall zwischen erstem Auftreten der Anfälle und der Operation, jedoch erwies sich die frühzeitige Operation als wesentlich zur Erreichung besserer und rascherer Rehabilitation. SUMMARYA specific direct gas chromatographic method to determine carbamazepine and, semiquantitatively, 10,11‐epoxy carbamazepine in serum is described. The average recovery of carbamazepine is 98%, and the error on duplicate determination is ±4%. The method is compared with Herrmann's classic spectro‐photometric method. In material of 103 patients the mean serum concentration of carbamazepine was 25.5 ± 12.8 μmoles/1 with GLC and 23.0 ± 12.6 μmoles/1 with spectrophotometry. The difference was highly significant. The blood sample volume is one‐tenth of that needed in spectrophotometry.RÉSUMÉUne méthode utilisant la chromatographie en phase gazuese est décrite pour le dosage spécifique de la carbamazépine et l'évaluation semiquantitative du 10,11‐époxy carbamazépine. Le taux moyen de carbamazépine retrouvée est de 98% et l'erreur sur des déterminations réalisées en double est de ± 4 %. La méthode est comparée à la méthode, spectrophotométrique classique de Herrmann.
