Summary:Phenobarbital (PB), diphenylhydantoin (PHT) and carbamazepine (CBZ) levels were determined in spiked rat brain homogenates by means of the EMIT® assay, and these were compared with results obtained by high‐pressure liquid chromatography (HPLC). The extraction procedure of the three drugs was identical for both methods, with minor modifications for EMIT (no addition of internal standard; reconstitution with the buffer used in routine plasma level analysis). Correlation coefficients between the data obtained by EMIT and HPLC were extremely good (PB, 0.90‐0.98; PHT, 0.92‐0.98; CBZ, 0.93‐0.99). The results indicate that the EMIT system can detect PB, PHT, and CBZ in brain tissue with good accuracy, although HPLC remains the most accurate and proven method available for research purposes.RESUMENSe han determinado los niveles de Fenobarbital (PB), difenilhidantoina (PHT) y carbamazepina (CBZ) en cerebros de ratas, homogeneizados mediante la técníca EMIT, y los resultados se han comparado con los obtenidos con cromatografía de líquidos a alta presión (HPLC). El procedimiento de extracción de las tres drogas ha sido idéntico para ambos métodos con mínimas modificaciones para la técnica EMIT (no incorporación de estandar interno; reconstitucion con el buffer usado en análisis de niveles plasmaticos rutinarios). Los coeficientes de correlacion entre la información obtenida con EMIT y HPLC fueron extremamente buenos (PB: 0.90‐0.98; PHT: 0.92‐0.98; CBZ: 0.93‐0.99). Los resultados indican que el sistema EMIT puede detectar PB, PHT y CBZ en tejido cerebral con bastante exactitud, aunque HPLC continúa siendo el método más exacto y verificado para propósitos de investigación.ZUSAMMENFASSUNGPhenobarbital‐(PB), Diphenylhydantoin‐(PHT) und Carbamazepin‐(CBZ) Spiegel wurden in “spiked” Homogenaten von Rattenhirn mit Hilfe der Emit‐Technik untersucht und die Ergebnisse mit denen der Hochdruckflüssigkeitschromatographie (HPLC) ver‐glichen. Die Extraktionsprozedur für die 3 Medi‐kamente war bei beiden Methoden identisch mit einer kleinen Modifikation für die Emit‐Methode (kein Hinzufügen eines internen Standards; Verwendung eines Puffers, wie er zur Routineanalyse der Plasma‐Spiegel benutzt wird). Die Korrelationskoeffizienten zwischen den Daten der Emit‐ und der HPLC‐Methoden waren äußerst günstig: (PB: 0,90‐0,98; PHT: 0,92‐0,98; CBZ: 0,93‐0,99). Summary:Valproic acid (VPA) was determined by EMIT® assay in plasma, tears, saliva, and cerebrospinal fluid (CSF) of patients with epilepsy. Closer correlation was shown between tear/plasma and tear/CSF ratios than between saliva/plasma and saliva/CSF ratios. The VPA CSF/serum ratio was in good agreement with data in the literature. Salivary levels were extremely erratic, while those for tears were much more reliable. Determination of VPA in tears is therefore the best method of studying the VPA free fraction in those cases in which investigations of protein binding of the drug are necessary.RESUMENCon el procedimiento EMIT se ha determinado el ácido valpróico (VPA) en plasma, lágrimas, saliva y líquido cefaloraquídeo (LCR) en pacientes con epilepsía. Se encontró una correlatión más estrecha entre lágrima/plasma y lágrima/LCR que entre saliva/plasma y saliva/LCR. La relación LCR/suero de VPA estaba de acuerdo con la informatión aparecida en la literature. Los niveles salivares fueron extremadamente er‐raticos mientras que los de las lágrimas eran más fiables. La determinatión del VPA en las lágrimas constituye el méjor metodo para el estudio de la fractión libre de VPA en los casos en los que, investigaciones sobre el acoplamiento de la droga (binding) a las proteínas, resultan necesarias.ZUSAMMENFASSUNGValproinsäure wurde mit der Emit‐Methode im Plasma, in der Tränenflüssigkeit, im Speichel und im Liquor cerebrospinalis (CSF) von Patienten mit Epilepsie bestimmt. Eine engere Korrelation zeigte sich zwischen Tranenflussigkeit und Plasma sowie Tränenflüssigkeit und CSF als zwischen Speichel und Plasma bzw. Speichel und CSF. Das Verhältnis des VPA im CSF und im Serum entsprach den Daten der Literatur. Die Speichelwerte waren aüßerst regellos, wahrend solche der Tränenflussigkeit sich als viel verlässlicher erwiesen. Die Bestimmung des VPA in der Tränenflüssigkeit ist deshalb die beste Methode für die Untersuchung der freien Fraktion des VPA in den Fällen, in denen Untersuchungen der Eiwei

Summary:Epilepsy is probably an inherited disorder. Animal studies, clinical studies on relatives of epileptics, and the study of the EEG both in patients and their relatives have provided evidence for this. Different specific modes of inheritance have been discussed. Since the HLA system offers one of the best sets of human genetic markers found so far, our aim was to investigate the occurrence of HLA antigens and haplotypes in children with different types of primary epilepsy and their families. The sample consisted of 53 mentally normal Caucasian children with epilepsy, 104 parents and 72 siblings. The types of epilepsy in the probands were divided into “benign epilepsy of childhood” (21), absences (18) in the haplotype study, comprising 51 families (two children were excluded), tonic‐clonic seizures (9), and complex partial seizures (5). With regard to HLA antigens, there was no difference between probands and their parents, respectively, and a control sample. Compared with the occurrence of the most common haplotypes in controls, there was a statistically significant low incidence of the haplotype A1,B8 in probands and parents for benign epilepsy of childhood, as well as for the total material. Due to its common occurrence in the general population, the haplotype A1,B8 must offer some kind of biological advantage, for example, carrying “protecting” genes against certain noxious mechanisms. Lack of this haplotype may explain a lowered immunologic defense mechanism, resulting in increased risk for the appearance of seizure disorders. A comparison of the HLA haplotype distribution among siblings and probands in the 17 families with more than one affected child suggested dominant inheritance, most evident for benign epilepsy of childhood. However, since the frequency of no shared haplotypes is fairly high, the results must be interpreted cautiously. Investigations with HLA typing in larger populations of epileptic individuals and their families may give a more reliable and conclusive answer regarding the genetic character of epilepsy.RESUMENLa epilepsía es, probablemente, un trastorno heredi‐tario. Esta afirmación se ha evidenciado en estudios en animates y chínicos, en los familiares de los epi‐lépticos y en el análisis de los EEG de los pacientes y familiares. Se han propuesto diversos modos es‐peci'ficos de herencia.Puesto que el sistema HLA es uno de los mejores marcadores de la genética humana, se ha intentado investigar la presencia de antígenos HLA y haplotipos en nin̄os con diversos tipos de epilepsía primaria y en hsus familias. El material consiste en 53 nin̄os normales con epilepsía, 104 padres y 72 hermanos. Los tipos de epilepsía en los probandos se dividieron en: “epilepsia benigna de la infancia” (21), ausencias (18 en en es‐tudio de haplotipo, que constó de 51 familias con dos nin̄os excluidos), ataques tónico‐clónicos (9) y ataques parciales complejos (5). Con respecto a los anti'genos HLA no se observaron diferencias entre los probandos y sus padres con respecto al grupo control. Se encon‐tro una reducida incidencia del haplotipo Al, B8 es‐tadisticamente significativa en los probandos y padres en el grupo “epilepsía benigna de la infancia,” asi como en el grupo total, con respecto a la presencia de los haplotipos más comunes en los controles. Debido a la frecuente existencia del haplotipo Al, B8 en la pob‐lación general, debe ofrecer algún tipo de ventaja biológica como por ejemplo, poseer genes protectores contra ciertos mecanismos nocivos. La falta de este haplotipo puede explicar una reducción de los mecanismos inmunológicos defensivos que resultan en un incremento del riesgo de la aparici6n de un tras‐torno comicial. La comparación de la distribución del haplotipo HLA entre los hermanos y los probandos en 17 familias, con más de un nino afecto, ha sugerido la existencia de una tendencia hereditaria más evidente para la “epilepsia benigna de la infancia”. Sin embargo, puesto que la frecuencia de haplotipos no com‐partidos es relativamente elevada, los resultados deben ser interpretados con cautela. Las inves‐tigaciones sobre la tipificación del HLA en amplias poblaciones de individuos epilépticos y sus familias pueden otorgar respuestas más fiables y conclusivas, en lo que concierne al estado geńetico de la epilepsía.ZUSAMMENFASSUNGDie Epilepsie ist wahrscheinlich eine vererbbare Storung. Dafür sprechen Tieruntersuchungen, klinische Untersuchungen von Verwandten Anfallskranker und die Untersuchungen des EEG sowohl bei Patienten wie ihren Verwandten. Un‐terschiedliche Erbgänge wurden diskutiert. Da das HLA‐System eines der besten “sets” genetischer Marker für den Menschen bietet, lag die Absicht nahe, das Auftreten von HLA Antigenen und Haplotypen bei Kindern mit unterschiedlichen Arten primarer Epilepsien und ihren Familien zu untersuchen. 53 kaukasische, geistig normale Kinder mit Epilepsie, 104 Eltern und 52 Geschwister wurden untersucht. Die Epilepsietypen der Probanden wurden unterteilt in be‐nigne Epilepsie des Kindesalters (21), Absencen (18 in der H'aplotypuntersuchung, entsprechend 51 Familien, 2 Kinder wurden ausgeschlossen), tonisch‐klonische Anfalle (9), komplexe Partialanfalle (5). Hinsichtlich HLA‐Antigenen existierten keine Unterschiede zwischer Probanden und ihren Eltern und den Kontrollen. Verglichen mit dem Auftreten der haufigsten Haplotypen bei den Kontrollen, traten bei alteren Probanden mit benigner Epilepsie des Kindesalters ebenso wie im gesamten Material statistisch signifikant seltener Haplotypen Al, B8 auf. Entsprechend ihrem allgemeinen Vorkommen in der Bevolkerung miissen die Haplotypen Al, B8 irgendeine Art biologischen Vorteil bieten, z.B. Protektorgene gegen bestimmte schadliche Mechanismen tragen. Das Fehlen dieser Haplotypen konnte einen verminderten immunologis‐chen Abwehrmechanismus kennzeichnen, der filr das erhohte Risiko einer Anfallskrankheit verantwortlich ist. Der Vergleich der HLA Haplotypen unter Gesch‐wistern und Probanden der 17 Familien, in denen mehr als 1 Kind erkrankt ist, lassen eine dominante Vererbung ganz besonders für die benigne Epilepsie im Kindesalter vermuten. Da jedoch die Haufigkeit verschiedener Haplotypen ziemlich hoch ist, miissen die Ergebnisse vorsichtig interpretiert werden. Summary:Epileptic activities induced by topical application of tranexamic acid (AMCA) and penicillin to the cortex of 12 cats in acute experiments were compared. Both substances when diffusely applied on a wide cortical area of both hemispheres at very low concentration produced an EEG pattern consisting of spike‐wave bursts similar to the electrographic manifestations seen in feline generalized epilepsy induced by large parenteral doses of sodium penicillin. These epileptic bursts could be triggered by repetitive stimulation of nucleus centralis medialis. Increased concentrations of both AMCA and penicillin led to the appearance of bilaterally synchronous spikes and poly‐spikes which were not further excited by NCM stimulation. Two factors seem to play an important role in eliciting spike‐wave bursts in both models: (1) the area of the cortex exposed to the epileptogenic agent and (2) the concentration of the epileptogenic agent used. The similar effects observed in 5 chronic animals either by intravenous injection of high doses of AMCA or by intramuscular injection of sodium penicillin confirm the results obtained in acute experiments and suggest a new way of inducing feline generalized epilepsy.RESUMELes AA. comparent les activités épileptiques pro‐voquées par l'application topique de AMCA et de Pénicilline sur le cortex de 12 chats en experimentation aigile. Ces substances–appliquées à une concentration très faible sur une surface étendue de deux hemispheres cérébraux–provoquent la apparition sur le tracé EEG de décharges de pointes‐ondes superposa‐bles aux tableaux observós dans l'épilepsie generalisee feline induite par doses parenterales importantes de Na Ṕenicilline. Ces decharges epileptiques peuvent etre induites par la stimulation repetitive du nucleus centralis medialis. L'augmentation de concentration soit de AMCA que de Pónicilline fair paraitre pointes et polipointes bilaterales et synchrones, qui ne peuvent plus etre induites par stimulation de NCM. Deux fac‐teurs semblent jouer un rdle important pour 1'induction de decharges de pointes‐ondes dans lex deux modeles: 1) la largeur de la surface du cortex exposée ä l'agent épileptogène; 2) la concentration de l'agent épileptogène employé. La similarité des effects observés dans cinq (5) animaux chroniques à la suite de l'injection de hautes doses de AMCA et de Na Penicilline confirme les résultats des expérimentations aigües et suggère une nouvelle voie pour induire l'épilepsie généralisée féline.RESUMENSe comparó la actividad epileptica inducida por la aplicacion tópica de ácido tranexámico (AMCA) y penicilina sobre la corteza de 12 gatos en experimentos agudos. Se aplicaron ambas substancias de una ma‐nera difusa a amplias zonas de los dos hemisferios y a bajas concentraciones se vio que inducían en el EEG descargas de punta‐onda similares a las vistas en la epilepsía generalizada del gata cuando se aplican grandes dosis de penicilina sódica por via parenteral. Estos paroxismos epilepticos podian ser desen‐cadenados por estimulación repetitiva del niicleo cen‐tromedial. Aumentando las concentraciones de AMCA y de penicilina aparecieron puntas y polipuntas sincronicas y bilaterales que ya no eran observables por estimulacibn del nucleo centromedial. Dos fac‐tores parecen jugar un papel importante en la aparicibn de descargas de punta‐onda en ambos modelos: (1) el area de corteza expuesta al agente epileptogdnico, (2) la concentracion del agente usado. Los efectos similares vistos en 5 animales cronicos tanto despues de la inyecci6n intravenosa de dosis altas de AMCA como de la inyección intramuscular de penicilina sbdica confirman los resultados obtenidos en experimentos agudos y sugieren una nueva via de inducir epilepsia generalizada en felinos.ZUSAMMENFASSUNGVergleich der epileptischen Aktivität, die durch to‐pische Applikation von Tranexamsaure (AMCA) und Penicillin auf den Kortex von 12 Katzen im akuten Experiment hervorgerufen wurden. Bei diffuser Anwendung auf ein großes cortikales Areal beider Hemispharen in sehr niedriger Konzentration waren beide Substanzen fahig, ein EEG‐Muster hervorzuru‐fen, das aus spike and wave bursts bestand; es ent‐sprach dem, das man bei generalisierter Epilepsie durch groBe parenterale Dosen von Natrium‐Penicillin bei Katzen beobachten kann. Diese epileptischen Ausbriiche konnten durch repetetive Stimulation des Nucleus centralis medialis getriggert werden. Die erhöhten Konzentrationen sowohl von AMCA als von Penicillin filhrten zum Auftreten bilateraler syn‐chroner spikes und poly spikes, die durch Stimulation des NCM nicht mehr auszulosen waren. In beiden Modellen scheinen zwei Faktoren eine wichtige Rolle in der Auslosung von spike‐wave bursts zu spielen: (1) Das Cortexareal, das mit dem epileptogenen Stoff in Berilhrung kommt; (2) Die Konzentration des epileptogenen Stoffes. Die ähnlichen Wirkungen, die bei 5 chronischen Tieren beobachtet wurden, die entweder eine i.v. Injektion von hoher Dosis AMCA oder eine i.m. Injektion von Natrium‐Penicillin bekamen, bes‐tatigen die Ergebnisse, die in akuten Experimenten gewonne
