Arterial reconstructive surgery is the alternative treatment for those impotent patients who wish the restoration of a normal erection without any mechanical or pharmacologic assistance. Although various operative approaches have been performed since the early 1970s, contemporary procedures are based on modern concepts of the physiology of erection, new diagnostic procedures in the dynamic state and endothelium-sparing vascular and microvascular principles. Success rates have reached 70% in selected patients.
