Emphasis in the acute postinfarction period should be on prevention of cardiac arrest rather than resuscitation; immediate recognition and vigorous treatment of potentially lethal arrhythmias is the key step. Better understanding of which arrhythmias are most hazardous has come from intracardiac recording of His bundle activity, a technique that can localize the site of conduction disturbances more precisely than the ECG.
