Due to the increasing concern for transmission of transfusion-associated infectious diseases, the incidence of autologous blood donation in pregnancy has increased. The authors retrospectively identified 273 consecutive third trimester pregnant blood donors who predonated with usual monitoring precautions, save for fetal monitoring, including a requirement for a minimum Hgb/Hct of 11 g%/34%, an interval of 5–7 days between donations, and no closer than 3 days before surgery. Data elicited included donation reactions and hospital course during subsequent delivery. 268 donors donated 341 units. Seven (2.1%) had mild reactions which included pallor, diaphoresis, nausea, lightheadedness, tachypnea and tachycardia. The incidence was not different from that seen with usual homologous blood donations. In 14 patients who had external fetal monitoring there were no fetal problems save for one deceleration episode which was positionally related.
