Renal Trauma in Children: A Ten ‐Year Review

The experience of renal trauma at the Royal Children's Hospital, Melbourne, is analysed. In assessing the 78 children involved, Nunn's (1962) classification into contusion (Type 1), cortical lacerations (Type 2), calyccal lacerations (Type 3) and complete tears (Type 4) was extended to include a fifth category, where the main damage was to the renal vessels. The clinical features and the radiological and operative findings were used to classify the lesions. Palls and motor vehicle accidents were the most common causes of injury. The examination of the methods of management indicates that in Types 1 and 2 non‐operative methods are usually sufficient; in Type 3 initial non‐operative management is indicated, but long‐term follow up is essential; in Type 4 injuries and vascular lesions the need for early diagnosis is paramount, and most patients will fare best with early surgical intervention.
