&NA;A major objective of the Chronic Care Networks for Alzheimer's Disease project is to involve and support family caregivers throughout the course of their relative's illness. The project model includes various approaches to achieve this objective, including innovative methods of involving family caregivers in the identification of people with possible dementia, initial assessment, and ongoing medical and non‐medical care management. Approaches and tools developed to implement the project model may be useful to other individuals and groups trying to create more coordinated and appropriate care for people with dementia and their family caregivers.
