When reflux persists despite aggressive medical management, surgery is indicated to repair a sliding hiatal hernia. Reflux may, however, recur postoperatively unless a competent closure mechanism is established at the gastroesophageal junction. The key step in the procedure, which is demonstrated, is to anchor the junction to the median arcuate ligament so that it remains permanently in its subdiaphragmatic position.
