Alterations in the symptom pattern of previously recognized heart disease or the sudden appearance for the first time of disabling cardiac symptoms define a preinfarction state that mandates prompt intervention. In many patients the treatment of choice is myocardial revascularization by saphenous vein bypass. Criteria for selection, steps in the work-up, and experience in this group of patients are detailed.
